Premise Health is the organization that operates and staffs the health centers. We’re one of the nation’s leading providers of direct healthcare, and we’re here as your health ally to help keep you at your best. We do this with experience, and shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

To contact the LP Health and Wellness Center regarding your appointment, you can call our LP Member Services department directly at (833) 930‐4539, stop by your local health center during regular business hours or see your nurse at an upcoming mill visit. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records can be available via Epic’s Care Everywhere platform and vice versa if you elect to opt in to that function in your MyPremiseHealth account.
  • $0 for all preventative services and visits for all employees and dependents enrolled in a LP-Sponsored medical plan
  • $50 per visit for employees who are not enrolled in a LP-Sponsored medical plan

The LP Health and Wellness Center provides care for all LP Building Solutions Employees as well as their dependents age 2 and up who are enrolled in an employer-sponsored medical plan.

While walk-ins are gladly accepted when we can accommodate them, due to the format of the Health Center, the preferred method is to schedule an appointment ahead of time. Same-day appointments are typically available depending on the service being requested.
The LP Health and Wellness Center is staffed, managed and operated by Premise Health. Each staff member has been carefully screened and is qualified to deliver exceptional care. The LP Health and Wellness Center is not staffed or managed by LP Building Solutions or their employees in any way.

My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

If you have not done so already, first set up your account on the My Premise Health app or online at mypremisehealth.com. Then, log in and select “Get Care Now.” A Premise Health team member will then put you in touch with your provider to begin your visit.
To meet virtually with one of the providers at your wellness center, you must first set up an account on the My Premise Health app or online at mypremisehealth.com if you haven’t already. Then, log in and select “Schedule an appointment.” Choose “Assisted Virtual” as your visit type and then select your appointment type that says “Video” or “Phone” next to it. You will then follow the prompts to complete scheduling.
A virtual visit it is just like in-person appointment. You can discuss your medical needs with your provider, and they will diagnose and prescribe medications as needed.
Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the LP Health and Wellness Center.
Yes, you can request that your PCP share your records with your wellness center by completing your provider’s record sharing process. You can also transfer your records and designate a provider at your wellness center as your new PCP.

Preventive care includes routine checkups and screenings that help you stay healthy in the long-term. These are visits you make when you’re feeling well to catch any health issues early or improve your wellbeing. This includes annual physicals, cancer screenings, vaccinations and more.

Non-preventive care on the other hand, includes visits you schedule when you’re not feeling well. These are meant to address your symptoms to find out what’s wrong and get you back to feeling your best. Some examples include sick visits and treatment for issues like cold and flu, allergies, injuries and more.
