Who is Premise Health?
How do I make an appointment?
You can call a center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

Health Center:
Monday – Friday: 8 a.m. – 6 p.m.

Will my current medical provider have access to my health records if I receive care at your center?
After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
How much does a visit cost?
Rock Health Collective in-person primary care visits:
- Rocket PPO Plan – $5 copay
- Rocket HDHP plan – $25 copay ($5 copay after deductible has been met)
Rock Health Collective virtual primary care visits:
- Rocket PPO Plan and Rocket HDHP Plan – $5 copay
Rock Health Collective virtual care management visits:
- Rocket PPO Plan and Rocket HDHP Plan – FREE
Rock Health Collective pharmacy:
- Generic – FREE
Not on a Rocket medical plan? No problem! You are still welcome to use the RHC for $25 a visit. Pharmacy costs will be subject to your specific pharmacy insurance plan.
Who is eligible to use the Rock Health Collective?
Rock Health Collective is open to all Quicken Loans team members and their eligible dependents.
What is My Premise Health?
My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.
