How do I make an appointment?
You can call a center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

How much does a visit cost?
All services at the SIA Health & Wellness Center, SIA Occupational Health Clinic, SIA Recreation Center are provided at no cost.
Who is eligible to use the wellness centers?
SIA Associates, spouses, and their dependents aged 10 and older on the SIA Health Plan are eligible. Eligible retirees under the age of 65 are also able to use the SIA Health & Wellness Center and Recreation Center.
Will my health information be secure?
Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. The Health & Wellness Center operates the same as any primary care office in the community. Your information is not shared without your consent. Your information will not be shared with the Occupational Health Clinic inside the plant. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.
Are same-day appointments available?
In most cases, same-day appointments are available. Schedule online or call your wellness center to see if a same-day appointment is available.
Are appointments required?
While walk-ins may be able to get an appointment, the preferred method is to schedule an appointment ahead of time. Same-day appointments are typically available.
How do I schedule a virtual visit with my onsite provider?
To meet virtually with one of the providers at your wellness center, you must first set up an account on the My Premise Health app or online at mypremisehealth.com if you haven’t already. Then, log in and select “Schedule an appointment.” Choose your appointment type that says “Video” or “Phone” next to it. You will then follow the prompts to complete scheduling.
Can my provider write me a prescription during a virtual visit?
A virtual visit it is just like in-person appointment. You can discuss your medical needs with your provider, and they will diagnose and prescribe medications as needed.
What is My Premise Health?
My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.
How is provider-filled medications different than a pharmacy?
With provider-filled medications, if your medication is on formulary, you receive your medications directly from your provider during the same visit in which they prescribe them. This is unlike a pharmacy, where pharmacists give you your prescriptions that were prescribed by your provider. You also get your medications at no cost, which is not often the case at a pharmacy.
Our providers can only dispense medications at in-person appointments, not virtually.
