Premise Health is a provider of in-person and virtual health for the world’s leading organizations. In other words, we partner with companies to bring their employees healthcare services and operate their wellness centers.

Members are what we call our patients who are eligible to use a wellness center operated by Premise Health. It’s not a membership that you have to pay for or apply to be a part of. You’re simply considered a member once your employer has partnered with us to provide your health and wellness services.

A wellness center is where you receive care as a member. We use this term versus the typical “doctor’s office” because we offer so much more than the usual office environment. Our centers not only typically offer more services than the average doctor’s office, but are also designed with you in mind, so you feel comfortable, calm and welcomed.

My Premise Health is your member portal. It is a secure online and mobile tool that you can use to manage your health information. My Premise Health connects you to your health by providing convenient access to book and manage appointments, message your providers, conduct virtual visits, view visit history, see your test results and more.

Eligibility and cost

Confidentiality

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within [Wellness Center Name].

Yes, you can request that your PCP share your records with your wellness center by completing your provider’s record sharing process. You can also transfer your records and designate a provider at your wellness center as your new PCP.

Scheduling

Once you create a My Premise Health account, you can easily schedule a visit through the My Premise Health app or online at MyPremiseHealth.com. Simply log in, select “Schedule an Appointment” and choose your appointment type.

You can also call your wellness center or visit during regular center hours to make an appointment.

In most cases, same-day appointments are available. Schedule online or call your wellness center to see if a same-day appointment is available.

While walk-ins are gladly accepted, the preferred method is to schedule an appointment ahead of time.

Virtual visits

If you have not done so already, first set up your account on the My Premise Health app or online at mypremisehealth.com. Then, log in and select “Get Care Now.” A Premise Health team member will then put you in touch with your provider to begin your visit.

To meet virtually with one of the providers at your wellness center, you must first set up an account on the My Premise Health app or online at mypremisehealth.com if you haven’t already. Then, log in and select “Schedule an appointment.” Choose your appointment type that says “Video” or “Phone” next to it. You will then follow the prompts to complete scheduling.

A virtual visit it is just like in-person appointment. You can discuss your medical needs with your provider, and they will diagnose and prescribe medications as needed.

Virtual visits are designed to treat your short-term needs and provide long-term preventive care with convenience and flexibility. If you’re experiencing a medical emergency, please dial 911 as soon as possible to get the medical attention you need.

Pharmacy

Also, our pharmacists go above and beyond what you experience at your typical pharmacy. They take the time to get to know you and work with your primary care provider to support your overall health. We also offer a wide range of services that go beyond simply prescribing medications, such as: [personal health counseling, coaching and guidance, condition management and disease or medication-specific education], as well as easy prescription transfers and low-cost generics.

Services

Preventive care includes routine checkups and screenings that help you stay healthy in the long-term. These are visits you make when you’re feeling well to catch any health issues early or improve your wellbeing. This includes annual physicals, cancer screenings, vaccinations and more.

Non-preventive care on the other hand, includes visits you schedule when you’re not feeling well. These are meant to address your symptoms to find out what’s wrong and get you back to feeling your best. Some examples include sick visits and treatment for issues like cold and flu, allergies, injuries and more.

Yes, your wellness center can perform blood draws and other lab tests ordered by an outside provider. All lab orders must be faxed or dropped off at the center to ensure we have the proper supplies for the required testing.
