Sally Keller, Health Center Manager, RN, MSN
Sally Keller RN, MSN graduated from the University of Washington School of Nursing in 1981. While in Seattle, WA her focus was in medical/oncology. In 1992 she obtained a Master of Science Degree in Community and Public Health Nursing from the University of Tennessee, Memphis. She worked with the Metropolitan Health Department of Nashville and Davidson County in primary care, homeless and infectious disease clinics. Sally joined Viatris Family Health Center in 1996 and enjoys reading, traveling, and spending time with her family.

Lavi Rohatgi, Medical Director, MD
Dr. Rohatgi has been Viatris Family Health Center’s Medical Director since 2008. She attended R. G. Kar Medical School in Calcutta, India and completed residency at St. John’s Hospital and Medical Center in Detroit, Michigan (2003). She worked for three years as a family practice physician at Mercy Hospital in Port Huron, Michigan then as a hospitalist at Northwestern Medical Center in St. Albans, Vermont for 3 years. She is board certified in Family Medicine and is certified as a CDL examiner. She has a daughter in college and lives with her husband in Williston.

Rebecca “Elaine” Taylor, MSN, NP-C
Elaine is a board-certified nurse practitioner from Morgantown WV, where she attended West Virginia University and graduated with her Master of Science in Nursing in 2010. She is licensed to practice in West Virginia, Pennsylvania, and Vermont. She has worked for Premise Health since 2016 at the Morgantown Mylan/Viatris location in the medical unit. Elaine Lives in Morgantown, WV with her husband Rusty and their four children.

Tammy Motoa, CCMA
Tammy graduated from Snead State Community College in Boaz, Alabama in 2015 as a Certified Clinical Medical Assistant and Certified Phlebotomist later receiving her National Certified Medical Assistant Certification in 2019. Tammy’s primary focus has been in Family Medicine and Occupational Medicine. Tammy enjoys cooking, decorating, candle making and spending time with family.

Theresa Ward, Receptionist
Theresa is a true native Vermonter. She was educated and held a successful career in accounting & finance for many years. After the birth of her son, she decided to pursue a career in the healing arts. She received multiple licenses and certifications from various institutions across the USA; including several from her favorite place; Santa Fe, NM. Before closing her private practice, she achieved medical certifications to work in medical administration. Most recently Theresa held the position of medical office manager. The office closed due to provider’s retirement. This led her to joining Viatris Family Health Center in 2023
