*All labs ordered by our Providers are at no cost and you will not receive an associated bill beyond the potential visit fee. All labs ordered by a community provider will have a visit fee associated and you may receive a bill from the ordering physician’s office or lab that processed your sample. The Health Center is strictly the blood draw station in this scenario and has no control over charges you may encounter.
For HRA/EPO accounts:
- All payments made for services will count toward your out-of-pocket max.
For HSA accounts:
- If you have not met your deductible, each visit is a flat $40 fee with no remaining balance due.
- If you have met your deductible, each visit is a flat $8 fee with no remaining balance due.
- Payments will count toward your plan deductible and out-of-pocket max.
All plans (HRA/EPO/HSA):
- Preventive care is covered at no cost to you. This includes your Annual physical, Well Woman visits and Vaccinations.
- Lab work ordered by our health center providers are covered at no cost to you.
- Lab work ordered by a community provider and obtained at the health center may result in a financial responsibility for the member.
- Fees are due at the time of service.
