Meghan Delgado, BSN, RN, Group Health Center Manager

Meghan Delgado is the Group Health Center Manager for Premise Health Pipe Trades Health & Wellness Centers. She has been a nurse for 16 years, starting her career as an LPN in 2007 before getting her RN in 2009. Meghan received her Bachelor of Science degree in Nursing in 2018 from St. Catherine University.  Her background is in Primary Care Leadership with a passion for holistic health and wellbeing. Collectively, Meghan has 13 years of Primary Care leadership experience and strives to create spaces that provide patients and visitors with a sense of belonging and community. 

Meghan is studying at St. Catherine University for her graduate degree in Holistic Health Studies and hopes to open doors and accessibility to holistic care and practices. She has worked with Premise Health for four years and believes that the organization has the ability to truly help others get, be and stay well. 

Dr. David Caccamo, Family Medicine/Medical Director

David Caccamo, MD, is the Medical Director at the Pipe Trades Health and Wellness Center.  After receiving his degree at Washington University School of Medicine in St. Louis, Missouri, he completed his residency training at Regions Hospital in St. Paul. He has practiced full-range Family Medicine in the Twin Cities east metro for over 25 years.  He enjoys caring for a broad range of people, from babies to seniors, and particularly developing ongoing care relationships with individuals and families over time.  

Dr. Caccamo has a particular interest in musculoskeletal medicine, skin problems, office procedures, as well as the evaluation and treatment of chronic medical conditions, including diabetes, hypertension, anxiety, depression and obesity.  He obtained additional Board Certification from the American Board of Obesity Medicine in 2021. 

During time outside of the office, he enjoys cooking, travel, Crossfit, disc golf and curling, along with spending time with his spouse and adult children.

Emily Bunt, NP-C

Emily is a family nurse practitioner with 20 years of experience. She has worked in a variety of settings such as family practice, geriatrics and nursing homes. Emily loves working with families and helping others reach their health goals through whole-person, tailored care. When not at work, Emily is with her two daughters and husband, either being active outside, enjoying going out to eat or traveling.

Fahmo Hans, LPCC, Behavioral Health Specialist

Fahmo Hans is a Licensed Professional Clinical Counselor (LPCC). She earned her Master’s Degree in Counseling and Psychological Services (MA) at Saint Mary’s University of Minnesota. Her approach to therapy is holistic and collaborative, with an emphasis on the client’s own strengths, skills and growth. Fahmo uses evidence-based interventions, primarily cognitive behavioral therapy, solution-focused, brief therapy and interpersonal psychotherapy. 

Fahmo believes that therapy can be challenging, and she strives to work at her clients’ pace to support them in meeting their goals. She utilizes a strengths-based framework and draws from several modalities to treat a wide range of mental health symptoms and conditions, such as anxiety, depression, ADHD, and relationship struggles.

Tara Smith, DC, Cac, Chiropractor

Dr. Tara Smith is a Board-Certified Doctor of Chiropractic. Originally from North Dakota, she attended college at NDSU where she got her Bachelor of Science degree in Human Performance and Fitness. After graduation she went on and got her Doctorate of Chiropractic degree from Northwestern Health Sciences University as well as her certification in Acupuncture. 

Dr. Smith looks at the whole body, not just the pain site, to find the true cause of the problem. To do this, she utilizes chiropractic care, acupuncture, cupping, scraping, connective tissue release, as well as muscle rehab and corrective exercises. Helping people feel their best is a passion for Dr. Smith for athletes and non-athletes alike. From newborns to the elderly, Dr. Smith takes a unique approach to chiropractic care. She uses a non-traditional, mechanical and functional based approach to overall body care and awareness to get you back to your pain-free life faster. She completes continuing education yearly to stay current on new advances in the chiropractic/athletic community and to keep on top of her game! 

 When Dr. Smith is not at work, she enjoys her time with her husband, Richard, and her two daughters, Ellie and Avery. 

Rochelle Aune, BS, MPT, Physical Therapist

Rochelle graduated with her bachelor’s in exercise science in 1995. While working as an EMT, she contributed to the rehab department, where she discovered her passion for physical therapy. Rochelle has more than 20 years of clinical experience in delivering results-driven care, with advanced training in postural restoration. She loves helping members return to their maximum functional ability, independence and pain relief so they can live life well. In her free time, Rochelle is always on the go with her husband and three children.
