Do you or a dependent have a complex, rare or undiagnosed condition that significantly impacts your daily life? Have you been unable to get answers from medical providers?

If so, you may be eligible for evaluation and treatment at Mayo Clinic—the world’s #1 rated hospital1—through the Care Excellence Program. This innovative collaboration between Premise Health, Mayo Clinic, and your wellness center is designed to help you and your dependents take charge of serious health issues. Participants will gain direct access to a world-class team of Mayo Clinic experts with experience in virtually every specialty and subspecialty.

The Mayo Clinic Complex Care Program makes it easier to get the care you need.

For eligible members, your Wellness Center will coordinate with Mayo Clinic to provide continuous care from start to finish. Services that you receive at Mayo Clinic through this program, including travel and lodging expenses, are covered by your health plan, with the exception of high deductible health plans.2

Premise Health’s Care Excellence program provides members access to the Mayo Clinic Complex Care Program, which offers enhanced benefits to eligible members facing complex, rare or undiagnosed health conditions. These individuals may be eligible to be referred to Mayo Clinic for evaluation and/or treatment. If you have a complex or rare condition, or you have been unable to find answers from other medical providers, the Mayo Clinic Complex Care Program may be able to help you get the care you need. Speak to your Premise Health provider to learn more about the program and eligibility.

Mayo Clinic experts are trained in virtually every medical specialty and subspecialty. If you have a complex or rare condition that significantly impacts your day-to-day life, or you have been unable to get answers from other medical providers, you may be a candidate for evaluation and treatment at Mayo Clinic. 

If a complex, rare or undiagnosed health condition is identified during your visit with a Premise Health provider, you may qualify for evaluation or treatment at Mayo Clinic. If medically appropriate, your provider will then request a referral to Mayo Clinic on your behalf.  

Your Premise Health team will work with Mayo Clinic to ensure your medical records and any other documents related to your health conditions are shared with the Mayo Clinic specialist reviewing your medical records. The Mayo Clinic specialist will determine if you need to travel to Mayo Clinic for further evaluation or treatment. If you decide to go to Mayo Clinic, travel and lodging for you and a caregiver will be arranged for you. After you return home, Premise Health and Mayo Clinic will work closely to coordinate your ongoing care.

Services that you or your covered family member receive at Mayo Clinic through this program are fully covered by your health plan after you’ve met your annual deductible. Covered services include travel and/or lodging expenses for you and one caregiver.

If your dependents have access to the wellness center, they are also eligible for the Mayo Clinic Complex Care Program. Contact your employer for more details. 

If your Premise Health provider submits a referral request to Mayo Clinic, you can expect a phone call within two business days. Once Mayo Clinic has all your medical records, a specialist will confirm that you qualify for the program, and Mayo Clinic will begin to coordinate your individualized appointment itinerary.
