We offer a comprehensive range of primary care services that focus on your long-term health, including sick and preventive care and management of your chronic conditions. Your primary care provider will get to know you so they are better equipped to prevent, treat and manage any health issues that may come up. The best part? You can schedule same-day, next-day and virtual appointments and message your provider using your secure member portal, My Premise Health. Women's health

We understand a woman’s healthcare needs are unique. Your provider will spend time getting to know you to best understand your health goals. They’ll come up with a personalized care plan to support your long-term health. Along with general primary care services, we offer the following specifically for women’s health:

  • Breast cancer screenings
  • Pelvic exams
  • Cervical cancer screenings (Pap smears)
  • Pregnancy testing
  • Comprehensive family care
  • Contraception (birth control) counseling

Care management

You don’t have to navigate your health challenges alone. With our care management services, you can get the support you need. This no-cost benefit provides a registered nurse as your care manager to be a go-to resource for all health-related questions, prescription guidance and more. They will come up with personalized treatment plans to help you make positive lifestyle changes to prevent, improve, manage or even reverse certain health issues.
Our treatment plans are tailored to meet your unique needs and can address a range of conditions, such as asthma, high blood pressure, obesity, diabetes and pre-diabetes, low back pain and more. We can provide one-on-one coaching, disease-specific education, self-care training, nutrition counseling, medication guidance and ongoing monitoring to manage your symptoms and help you stay well.

Mental health

We believe that your mental and emotional wellbeing are just as important as your physical health. With our mental health services, you can get the support you need. Your licensed counselor will actively work as part of your care team to support your overall health while making you feel safe and comfortable. You can conveniently meet with them in person or virtually, through video or by phone.

With a blended approach of counseling techniques and evidence-based talk therapy, our counseling services cover anxiety, depression, stress management, PTSD, ADHD, grief, adjustment challenges, substance abuse, work/life balance, family conflict and sleep issues.

Lab services

We offer many lab services to support your needs. We can perform onsite testing and can also collect and send samples to an offsite lab. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health account or during an in-person or virtual follow-up appointment with your provider.

Physical therapy

You don’t have to live with pain and discomfort. Our physical therapists are dedicated to your wellbeing. They work with your primary care provider to understand your specific health needs and conditions. They’ll get to the root cause of your injury and work with you to prevent future complications, so you can get back to moving with ease.

We offer a full range of in-person and virtual services, including on-the-job injury prevention and treatment, therapeutic exercise and conditioning, return-to-work programs, pre- and post-operation rehabilitation, ergonomics evaluations onsite or virtually, orthopedic and neurological rehabilitation, health management and awareness education, and job function testing.

Occupational health

Keeping you safe at work is a top priority, which is why we offer a full suite of occupational health services. Your providers are here to treat work-related injuries and illnesses to help you get back to feeling well.

Our occupational health services include injury diagnosis and treatment, emergency care services, musculoskeletal evaluations, ergonomic assessments, pre/post-employment exams, drug and alcohol testing, physical therapy and occupational therapy services and return-to-work plans.

Vaccines

Vaccines are an essential part of public health. You have convenient access to many vaccines at Fiserv Wellness Center. Your provider can determine which ones are right for you based on your age, conditions, travel habits and other factors.

We offer a variety of routine vaccines to protect your health and community, including:

  • Hepatitis B
  • Human Papilloma Virus (HPV)
  • Measles, Mumps and Rubella (MMR)
  • Pneumococcal (Pneumonia, Meningitis, etc.)

Care Navigation

Your wellness center offers many services to cover your health needs. However, there may be times you need specialty care outside the center. Finding that care on your own can sometimes be confusing. That’s why we provide care navigation to support you in finding the affordable, high-quality care you need.

If your provider gives you a specialist referral, your care navigator will help you find the best care at the best price in your community. They’ll even assist with scheduling your appointments, transferring your medical records and following up after your specialist visit to help with any ongoing care needs.

Care Excellence

At the City of Mesa Wellness Center, we make sure you’re never alone in making complicated decisions about your health. If you’re struggling to get a diagnosis or battling a rare, complex condition, your Premise Health provider can help. Our partnership with Mayo Clinic offers you access to specialized care.

If medically appropriate, your provider will refer you to the Mayo Clinic for evaluation and/or specialized treatment. Your Mayo specialist and your provider at your wellness center will work closely to coordinate your ongoing care.

If you decide to go to the Mayo Clinic, all travel and lodging are covered and conveniently arranged for you and one caregiver. All costs associated with the Mayo Clinic Care Excellence program are covered by your health plan unless you’re on a high deductible plan.

Care Consult

If you receive an abnormal test result, are told by a provider in your community that you need major surgery or experience some other serious health issue, you may benefit from a second opinion. Your providers can offer that through Premise Health’s partnership with Mayo Clinic. They’re able to virtually consult with specialists at Mayo to review your medical record and come up with a recommended treatment plan for your specific health concern. Your provider will then share the outcome of that conversation with you to help you navigate your care plan and any next steps.

Schedule a visit with your provider to get started. Should you need a second opinion from a specialist, your provider will facilitate that through care consult.

You can get the same great care you receive at your wellness center, virtually, either online or over the phone. This safe and convenient option gives you the flexibility to schedule a virtual appointment with your trusted provider during your center’s regular business hours, at the same cost as an in-person visit. Premise Health is the organization that operates and staffs your wellness centers. We are the nation’s leading provider of direct healthcare, which means we partner with companies to offer their employees high-quality in-person and virtual health services. We are here as your health ally to help you get, stay and be well. We do this with experience, and by shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

Members are what we call our patients. If your employer has partnered with Premise Health to provide access to our wellness centers and services, you’re automatically considered an eligible member. There is no membership fee to get started. It also reflects how much we value our care relationship with you, because to us, you’re more than just a patient – you’re a member.

Your wellness centers are staffed, managed and operated by Premise Health. The medical staff includes a combination of nurse practioners, care management registered nurses, wellness coaches, physical therapists and medical assistants. Each staff member has been carefully screened and is qualified to deliver exceptional care. Your wellness centers are not staffed or managed by Fiserv or their associates in any way.

My Premise Health is your member portal. It is a secure online and mobile tool that you can use to manage your health information. My Premise Health connects you to your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.
