Everything you need in one place

Virtual Primary Care

You don’t have to coordinate your own care. With virtual primary care through the Delta Virtual Wellness Center, you have access to a dedicated provider by phone or video. It’s just like seeing a primary care provider (PCP) in person, but without the waiting room, commute or disruption to your day.

Skip the various appointments at different offices and trying to communicate with multiple providers. Instead, designate one of our virtual primary care providers as your PCP. They’ll become your trusted health ally and help you better prioritize your health.

You can receive the treatment you need for an illness, get a medication refill, discuss test results, develop a wellness plan and more with your provider. You can contact your care team 24 hours a day, no matter where you are in the U.S.

Care Management

Care Management offers guided clinical support when you need it most. Whether you’re juggling multiple chronic conditions or need a little extra help getting your health in check, a care manager is there and ready to walk alongside you every step of the way.

As a Delta employee or family member, you have a dedicated care manager available at no charge through your health plan. Your care managers are licensed nurses who work with your care team to provide personalized care plans, coaching, education, ongoing monitoring and follow-ups throughout your health and wellness journey. Through hands-on guidance and regular check-ins, care managers can help with a wide range of concerns and chronic conditions, including diabetes, cancer, Lupus, hypertension, mental health disorders, and any other health challenges that may require frequent touchpoints with your care team.

Care Navigation

Despite having access to a variety of services to support you in your health journey, there may be times when you require care outside of what’s offered. Finding the right specialist on your own can be difficult, which is why the Care Navigation program is available to help guide you to the affordable, high-quality specialty care you need, exactly when you need it.

If your virtual primary care provider determines you should see a specialist, one of Delta’s care navigators will reach out to help you find high-value specialty care within your community. That care navigator will also assist you by scheduling appointments, sending appointment reminders, transferring medical records and following up with you after your specialist visit(s) to help with any ongoing care needs.

Should you need assistance in finding specialty care, schedule a virtual visit with your primary care provider today. Once your provider determines the need for a specialist, a care navigator will contact you to assist with the next steps.

Care Consult

If you’ve recently received an abnormal test result, are told by a provider in your community that you need major surgery, and/or you experience a serious health issue, you may benefit from a second opinion. If medically appropriate, your virtual primary care provider can initiate this service at no charge to you through a partnership with Mayo Clinic’s network of industry-leading specialists and subspecialists.

Upon initiating an eConsult, your virtual primary care provider will consult with specialists at Mayo to review your medical record and determine a recommended treatment plan for your specific health concern. Your provider will then share the outcome of that conversation to help you navigate any next steps in your care plan.

Care Excellence

We strive to make sure you’re never alone in making complex decisions about your health. If you’re struggling to get a diagnosis or battling a rare, serious, and/or complex condition(s), your virtual primary care provider can help. Our partnership with Mayo Clinic offers you access to the world’s leading specialty and subspecialty care.  

If medically appropriate, your virtual primary care provider can refer you to the Mayo Clinic for evaluation and/or specialized treatment. Once treatment is complete, your Mayo specialist and virtual primary care provider will collaborate closely to coordinate your ongoing care. 

Should your provider recommend you seek treatment at the Mayo Clinic, Delta will cover the cost for all travel and lodging to Mayo’s nearest location, conveniently arranged for you and one caregiver through Mayo Clinic’s travel services team. All costs associated with the Mayo Clinic Care Excellence program outside of evaluation and travel/lodging costs are subject to deductibles, copays, and out-of-pocket maximums as outlined in your health plan. Occupational Health

Keeping you safe at work is a top priority, which is why we offer a full suite of occupational health services. Your providers at CarePort at Health Central are here to treat work-related injuries and illnesses to help you get back to feeling well.

Our occupational health services are available for all Delta employees and include injury diagnosis and treatment, musculoskeletal evaluations and physical therapy services.

Care Excellence

Care navigation

Members are what we call our patients. If your employer has partnered with Premise Health to provide access to our wellness centers and services, you’re automatically considered an eligible member. There is no membership fee to get started. It also reflects how much we value our care relationship with you, because to us, you’re more than just a patient – you’re a member.

Your virtual wellness center is staffed, managed and operated by Premise Health. The virtual wellness center’s clinical staff includes a team of physicians, nurse practitioners, care management registered nurses, and care navigators. Each staff member has been carefully screened and is qualified to deliver exceptional care. Premise providers are not staffed or managed by your employer in any way.
