We offer a comprehensive range of primary care services that focus on your long-term health, including sick and preventive care and management of your conditions. Your primary care provider will get to know you and your eligible dependents, so they are better equipped to prevent, treat, and manage any health issues that may come up.
Our lifestyle medicine approach ensures we’re looking at the whole picture of what impacts your wellbeing, including sleep, nutrition, stress, movement and more. We can encourage healthy habits to help you achieve sustainable wellness and go beyond simply treating the symptoms you bring with you to a single appointment.
Skip the extra trip to the pharmacy. You can get your medications at no cost directly from your provider during a visit. Simply schedule an appointment, discuss your health needs, and leave with your medication in hand.
CTK Health and Wellness Center carries close to 200 medications to cover your needs. This includes over-the-counter options. You can even get up to a 90-day supply for ongoing prescriptions to reduce the number of times you need a refill. When you’re about to run out, schedule a follow-up visit with your provider. They’ll monitor your progress and offer ongoing support.
We understand a woman’s healthcare needs are unique. Your provider will spend time getting to know you and your eligible dependents to best understand your health goals. They’ll come up with a personalized care plan to support your long-term health.

We offer many lab services to support you and your eligible dependent’s needs. We can perform onsite testing and can also collect and send samples to an offsite lab. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health account or during an in-person or virtual follow-up appointment with your provider.

Vaccines are an essential part of public health. You and your eligible dependents have convenient access to many vaccines at CTK Health and Wellness Center. Your provider can determine which ones are right for you based on your age, conditions, travel habits, and other factors.
Wellness coaching can help you achieve lasting lifestyle change in the areas of weight management, nutrition, physical activity, sleep, stress, smoking cessation, work/life balance, and life satisfaction. Our certified wellness coaches are trained specialists who support you as you move toward better health at a comfortable pace. They will help you set actionable goals and teach you new skills as you learn to incorporate healthier habits into your daily routine. All coaching sessions are completely confidential and can take place virtually.
Our wellness programs promote nutrition, physical activity, stress management, healthy relationships, and more. We take a comprehensive approach that focuses on your overall health and well-being. Our providers will personalize your wellness plan and help you and your eligible dependents make positive lifestyle changes that can lead to a more balanced and healthier you.
Your wellness services include biometric screenings, centralized virtual wellness coaching, and condition management.

Members are what we call our patients. If your employer has partnered with Premise Health to provide access to our wellness centers and services, you’re automatically considered an eligible member. CTK Health and Wellness Center is staffed, managed and operated by Premise Health. The medical staff includes a combination of physicians, nurse practitioners, nurses, medical assistants, and wellness coaches. Each staff member has been carefully screened and is qualified to deliver exceptional care. Your wellness centers are not staffed or managed by Kenton County or their employee’s in any way.
My Premise Health is your member portal. It is a secure online and mobile tool that you can use to manage your health information. My Premise Health connects you to your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

Kenton County employees and dependents age 5 and up on employer sponsored benefit plan are eligible to receive care at the three Northern Kentucky health centers.

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.
Yes, you may request any records be sent to another medical provider. If your community provider uses Epic (or My Chart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.

To meet virtually with one of the providers at your wellness center, you must first create an account on the My Premise Health app or online at mypremisehealth.com. Log in and select “Schedule an appointment,” choose your appointment type and if prompted, select whether you’d like to be seen by phone or video. A virtual visit is just like an in-person appointment. You can discuss your medical needs with your provider, and they will diagnose and prescribe medications as needed.

Preventive care includes routine checkups and screenings that help you stay healthy in the long-term. These are visits you make when you’re feeling well to catch any health issues early or improve your wellbeing. This includes annual physicals, cancer screenings, vaccinations and more.
Non-preventive care on the other hand, includes visits you schedule when you’re not feeling well. These are meant to address your symptoms to find out what’s wrong and get you back to feeling your best. Some examples include sick visits and treatment for issues like cold and flu, allergies, injuries and more.
Yes, your wellness center can perform blood draws and other lab tests ordered by an outside provider. All lab orders must be faxed or dropped off at the center to ensure we have the proper supplies for the required testing.

Monica was born in Cleveland and raised in Zanesville, Ohio. She went to college at Kent State University and Medical School at The University of Cincinnati College of Medicine where she discovered her passion for Family Medicine. She delights in being part of the journey with her patients and finding true joy in taking part in their care, treating the human first not the disease. She lives in the Cincinnati area with her husband for nearly 20 years, and two teenage children, keeping her busy and are the center of her universe.

Leslie is a board-certified Family Nurse Practitioner with 12 years of diverse experience in emergency care, preoperative services, and retail health. She earned her BSN from the University of Cincinnati and her MSN-FNP from Sacred Heart University. A proud member of the American Association of Nurse Practitioners, Leslie provides compassionate, patient-centered care across the lifespan, empowering individuals to manage both acute and chronic conditions with confidence. She values open communication and a holistic approach rooted in kindness and respect.
Outside of work, Leslie enjoys time with loved ones, traveling, rock climbing, and exploring the outdoors. She resides in the Cincinnati area with her husband and two young daughters.
Tennika Binius, LPN
Nurse Practitioner
Tennika is a dedicated Licensed Practical Nurse with over seven years of experience. Her nursing journey has spanned multiple settings, including hospital float teams, Long-Term Acute Care (LTAC), and travel nursing during the height of the COVID-19 pandemic. In 2021, she transitioned into a clinical environment, where she discovered her true calling. Since then, she has thrived in this setting and remains deeply passionate about her career in nursing.
Originally from Canton, Ohio, Tennika now resides in the Northern Kentucky/Cincinnati area, a move inspired by her husband’s career. She’s grateful for the unwavering support of her husband and three children, who have been with her every step of the way. She beams with pride for her beautiful daughter, her precious granddaughter, and her two fabulous sons, one of whom is currently in college. Tennika is thrilled to be part of Premise Health and looks forward to building strong connections with the team!

Chris was born and raised in Union, KY. She is an animal-loving farm girl at heart. Summer is her season when you will find her by the pool, beach or on a cruise when she can. When not by the water, you will find her enjoying digging in the garden, reading or listening to audiobooks or spending a night with my friends playing games.
Chris worked with Dr Sullivan for 4 years and for her directly for over a year and was heartbroken to learn that she was leaving. So, imagine her surprise when she found out that she had mentioned her to this great company and wanted her to come with her. So, follow she did, and now she is off on another adventure.

Kim was born and raised in Steubenville, OH and now resides in Independence, Kentucky. She received her Bachelor of Arts in Microbiology from Miami University and her Osteopathic Medicine degree from the Ohio University Heritage College of Medicine in Athens, OH. She previously worked at St. Elizabeth Hospital and started her own health and wellness practice. She works with patients at Florence Health & Wellness Center. She loves traveling with her family.
