Our pharmacy is a convenient way to fill your prescriptions. The pharmacists here are an extension of your care team, working with your provider as needed to support your overall health. We also offer a wide range of services that go beyond prescribing and filling medications, such as personal health counseling, medication-specific education, and coaching and guidance, plus easy prescription transfers and discounted over-the-counter medications.

Our physical therapists work closely with your primary care provider to understand your specific needs, treat the root cause of your discomfort and prevent future injuries. Musculoskeletal wellness exams are available at no cost, and members have direct access to physical therapy services for up to 30 days with a physical therapy order.

Whether you’re recovering from an injury, managing a chronic condition or dealing with everyday aches and pains, we’re here to help you move with ease and confidence. Our team will meet with you one-on-one for an initial evaluation to establish a plan of care for many diagnoses including (but not limited to):

We offer many lab services to support your needs. We offer full-service lab panels and can perform onsite testing or collect and send samples to an offsite lab. We accept community lab orders from outside providers to be drawn in-house at the Associate Family Health Center at no additional cost.

Our turnaround time for processing routine lab tests is generally 24 to 72 hours. You can receive your results electronically through your My Premise Health account or during an in-person or virtual follow-up appointment with your provider.
