How do I make an appointment?
You can call a center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.
Who is eligible to use the Employers’ Healthcare Center and Pharmacy?
The Employers’ Healthcare Center and Pharmacy is only available to those who receive the medical benefits from CNH, such as employees, dependents, and eligible retirees.

How much does a visit cost?
Services are subject up to a $50 co-pay for all non-client covered services.
Will my current medical provider have access to my health records if I receive care at the Employers’ Healthcare Center and Pharmacy?
After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
What is My Premise Health?
My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.
Is my health information secure? Does CNH have access to my personal health information?
Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the healthcare center.
Will my current medical provider or PCP have access to my health records if I receive care at Employers’ Healthcare Center and Pharmacy?
Yes, you may request any records be sent to another medical provider. If your community provider uses Epic (or My Chart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
How does my provider send my prescription to the Employers’ Healthcare Center and Pharmacy?
You can easily drop off your prescriptions at Employers’ Healthcare Center and Pharmacy or your provider can e-prescribe them directly to the pharmacy. The providers and pharmacy at your healthcare center act as one care team. Your provider will send your prescription to the pharmacist after your appointment so you can pick it up the same day, making for a seamless experience.
