Who is Premise Health?
How do I make an appointment?
You can call your center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

When is the Premise Health Center? open?
Monday, Tuesday, Thursday, Friday, 8 a.m. – 5 p.m.
Wednesday, 10 a.m. – 7 p.m.
Will my current medical provider have access to my health records if I receive care at one of your centers?
After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
How much does a visit cost?
Services are available for a $10 copay on the Physicians Health Plan. If you are on the High-Deductible Health Plan (HDHP) or Health Savings Account (HSA), you pay $35 per visit until your deductible is met.
Who is eligible to use the Premise Health Center?
If you have received communications from Premise Health, you and your enrolled dependents (ages 2+) are eligible for Premise Health services as part of your PHP benefits.

What is My Premise Health?
My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.
Are appointments required?
While walk-ins are gladly accepted, you may also schedule an appointment ahead of time. Same-day appointments are typically available.
