Who is Premise Health?
How do I make an appointment?
You can call a center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

Who is eligible to use the T-Mobile Onsite Health Center?
All T-Mobile employees, spouses and dependents ages 2+ on a company health plan are eligible to use the Health Center. Employees with waived coverage have access to limited services.
Can I do lab/blood test orders from another provider at the center?
Yes, your wellness center can perform blood draws and other lab tests ordered by an outside provider. All lab orders must be faxed or dropped off at the center to ensure we have the proper supplies for the required testing.
Will my current medical provider have access to my health records if I receive care at one of your centers?
After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
Who operates the T-Mobile Onsite Health Center?
T-Mobile Onsite Health Center is staffed, managed and operated by Premise Health. Each staff member has been carefully screened and is qualified to deliver exceptional care. T-Mobile Onsite Health Center is not staffed or managed by T-Mobile or their employees in any way.
What is My Premise Health?
My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.
Will my health information be secure? Will T-Mobile have access to my personal health information?
Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within T-Mobile Onsite Health Center.
I currently have a primary care provider (PCP) elsewhere. Can my medical records be shared between my wellness center and my current PCP?
Yes, you can request that your PCP share your records with your wellness center by completing your provider’s record sharing process. You can also transfer your records and designate a provider at your wellness center as your new PCP
What is the difference between preventive and non-preventive care visits?
Preventive care includes routine checkups and screenings that help you stay healthy in the long-term. These are visits you make when you’re feeling well to catch any health issues early or improve your wellbeing. This includes annual physicals, cancer screenings, vaccinations and more.
Non-preventive care on the other hand, includes visits you schedule when you’re not feeling well. These are meant to address your symptoms to find out what’s wrong and get you back to feeling your best. Some examples include sick visits and treatment for issues like cold and flu, allergies, injuries and more.
