How do I make an appointment?
You can call your center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

How do I schedule a virtual visit with my onsite provider?
To meet virtually with one of the providers at your wellness center, you must first set up an account on the My Premise Health app or online at mypremisehealth.com if you haven’t already. Then, log in and select “Schedule an appointment.” Choose your appointment type that says “Video” or “Phone” next to it. You will then follow the prompts to complete scheduling.
Will my current medical provider have access to my health records if I receive care at one of your centers?
After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
Can my provider write me a prescription during a virtual visit?
A virtual visit it is just like in-person appointment. You can discuss your medical needs with your provider, and they will diagnose and prescribe medications as needed.
Are appointments required or can I just walk in?
How much does a visit cost?
Fees depend on your plan, but they will likely be lower than a traditional, community-based healthcare provider.
What is My Premise Health?
My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.
Who is eligible to use the health centers?
All eligible Johnson & Johnson employees are welcome to use the Health Center.
Is my health information secure? Does Johnson & Johnson have access to my personal health information?
Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the health center.
Will my current medical provider or PCP have access to my health records if I receive care at the Health Centers?
Yes, you may request any records be sent to another medical provider. If your community provider uses Epic (or My Chart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
Can I do lab/blood test orders from another provider at the center?
Yes, your health center can perform blood draws and other lab tests ordered by an outside provider. All lab orders must be faxed or dropped off at the center to ensure we have the proper supplies for the required testing.
