Premise Health is the organization that operates and staffs the health centers. We’re one of the nation’s leading providers of direct healthcare, and we’re here as your health ally to help keep you at your best. We do this with experience, and shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

You can call a center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.

For members covered by a HealthSpring Consumer Driven High Deductible Plan (CDHP), there is a $20 charge for non-preventative services in Primary Care and Behavioral Health visits.
For members covered by other HealthSpring Open Access Plus plans (not high deductible plans), services provided at the EHC are free of charge.
Wake retirees covered by HealthSpring Medicare Advantage, as well as those covered by Medicare Part B, should consult their insurance ID cards for expected charges for EHC services.

Wake County Employee Health Centers are available to Wake County employees, spouses, and dependent children ages 18-26 who are enrolled in Wake County sponsored healthcare plans, as well as Wake County Retirees. Employees are not required to use sick leave for appointments at the Employee Health Centers.

To meet virtually with one of the providers at your wellness center, you must first set up an account on the My Premise Health app or online at mypremisehealth.com if you haven’t already. Then, log in and select “Schedule an appointment.” Choose your appointment type that says “Video” or “Phone” next to it. You will then follow the prompts to complete scheduling.

A virtual visit it is just like in-person appointment. You can discuss your medical needs with your provider, and they will diagnose and prescribe medications as needed.

My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within Wake County Employee Health Center.

Yes, you can request that your PCP share your records with your wellness center by completing your provider’s record sharing process. You can also transfer your records and designate a provider at your wellness center as your new PCP.

If you have not done so already, first set up your account on the My Premise Health app or online at mypremisehealth.com. Then, log in and select “Get Care Now.” A Premise Health team member will then put you in touch with your provider to begin your visit.
