General

Premise Health is a provider of in-person and virtual health for the world’s leading organizations. In other words, we partner with companies to bring their employees healthcare services and operate their wellness centers.

Members are what we call our patients who are eligible to use a wellness center operated by Premise Health. It’s not a membership that you have to pay for or apply to be a part of. You’re simply considered a member once your employer has partnered with us to provide your health and wellness services.

A wellness center is where you receive care as a member. We use this term versus the typical “doctor’s office” because we offer so much more than the usual office environment. Our centers not only typically offer more services than the average doctor’s office, but are also designed with you in mind, so you feel comfortable, calm and welcomed.

Foundation Medicine Occupational Health Center is staffed, managed and operated by Premise Health. Each staff member has been carefully screened and is qualified to deliver exceptional care. Foundation Medicine Occupational Health Center is not staffed or managed by FMI or their for employees in any way.

While where you receive care is a personal choice, we hope you’ll consider trusting us with your overall wellbeing, much like FMI has trusted us as a partner in health. We’d love to see you soon!

Eligibility and cost

All FMI employees are eligible to use the services at the Foundation Medicine Occupational Health Center.

Confidentiality

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. Yes, you can request that your PCP share your records with your wellness center by completing your provider’s record-sharing process.

Scheduling

Appointments can be scheduled by email at [email protected], phone (Boston: 857-242-7337 / Morrisville: 984-243-9581), or walk-in. If you’re experiencing a medical emergency and are working onsite, please dial x3333.

In most cases, same-day appointments are available. Email or call your wellness center to see if a same-day appointment is available.

While walk-ins are gladly accepted, the preferred method is to schedule an appointment ahead of time.
