Primary care

We offer a comprehensive range of primary care services that focus on your long-term health, including sick and preventive care and management of your conditions. Your primary care provider will get to know you so they are better equipped to prevent, treat and manage any health issues that may come up. 

Our lifestyle medicine approach ensures we’re looking at the whole picture of what impacts your wellbeing, including sleep, nutrition, stress, movement and more. We can encourage healthy habits to help you achieve sustainable wellness and go beyond simply treating the symptoms you bring with you to a single appointment.

Care management

You don’t have to navigate your health challenges alone. With our care management services, you can get the support you need. This no-cost benefit provides a registered nurse as your care manager to be a go-to resource for all health-related questions, prescription guidance and more. They will come up with a personalized treatment plan to help you make positive lifestyle changes to prevent, improve, manage or even reverse certain health issues. 

Our treatment plans are tailored to meet your unique needs and can address a range of conditions, such as asthma, high blood pressure, obesity, diabetes and pre-diabetes, low back pain and more. We can provide one-on-one coaching, disease-specific education, self-care training, nutrition counseling, medication guidance and ongoing monitoring to manage your symptoms and help you stay well.

Women's health

We understand a woman’s healthcare needs are unique. Your provider will spend time getting to know you to best understand your health goals. They’ll come up with a personalized care plan to support your long-term health. 

Along with general primary care services, we offer the following specifically for women’s health:

  • Breast cancer screenings
  • Pelvic exams
  • Cervical cancer screenings (Pap smears)
  • Pregnancy testing
  • Comprehensive family care
  • Contraception (birth control) counseling
  • STD testing and counseling
  • Education for self-awareness

Lab services

We offer many lab services to support your needs. We can perform onsite testing and can also collect and send samples to an offsite lab. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health account or during an in-person or virtual follow-up appointment with your provider.

Vaccines

Vaccines are an essential part of public health. You have convenient access to many vaccines at Wellness @ The Center. Your provider can determine which ones are right for you based on your age, conditions, travel habits and other factors.

We offer a variety of routine vaccines to protect your health and community, including:
  • Influenza (Flu)
  • COVID-19
  • Hepatitis A
  • Hepatitis B
  • Human Papilloma Virus (HPV)
  • Measles, Mumps and Rubella (MMR)
  • Pneumococcal (Pneumonia, Meningitis, etc.)
  • Tdap/Td (Tetanus, Diphtheria and Pertussis / Whooping Cough)
  • Zoster (Shingles)

Pharmacy

Pharmacy @ The Center is conveniently located within the Vermeer Health Services building to get your prescriptions to you faster and easier. Our pharmacists take the time to get to know you and work with your primary care provider to support your overall health. They’ll not only fill your prescriptions but will work with you to manage your medications and conditions.

Unlike your usual big-box pharmacy, we offer a wide range of services that go beyond simply prescribing and monitoring medications. We can provide personal health counseling, and disease or medication-specific education, as well as easy prescription transfers and low-cost generics for you and eligible dependents. We also offer mail order and auto-refill prescription services. Ask your pharmacist about how to get signed up.

Travel medicine

At Wellness @ The Center, we offer travel medicine services to help you and your family plan for your trip and to stay well while you’re away. Based on your destination, things like getting vaccinated, refilling your prescriptions and discussing health risks with your provider can make or break your travel experience.

Our travel medicine services include pre-travel risk assessment, prescriptions and vaccinations based on destination, risk reduction planning and education for self-treatment, as well as post-travel assessments and treatment, or specialist referral.

Our secure member portal makes it easy to connect with your care team. Whether you’re at home or on the go, you can schedule appointments, message your providers, view your lab results and more – anytime, anywhere.

Get the same great care by phone or video.

You can schedule a virtual appointment with your trusted care team during our regular business hours, at the same cost as an in-person visit.

Meet your care team

Jen West, RN, Health Center Manager

Jen West is the health center manager for Vermeer. She graduated from Mercy College with an associate degree in nursing in 2007. She began her healthcare career at Mercy Hospital as a float team member, where she was able to learn and grow in a variety of different care settings. Since that time, her primary background has been in nursing leadership, education and compliance. Jen joined Premise and Vermeer in 2017 and has been able to see services grow from acute episodic care to the much fuller range of services offered today. Through the partnership with Vermeer, she is able to live out her passion for caring for others while also helping them invest in themselves. She resides locally, in Pella, with her husband and two daughters. Jen is an avid animal lover and enjoys reading and hiking.

Ann Bartels, Nurse Practitioner

Ann Bartels is a nurse practitioner in primary care with Premise Health at Vermeer. She has been with Premise Health since 2020. Ann graduated with her Bachelor of Science in nursing in 2003 from Allen College and Master of Science in nursing with family nurse practitioner in 2010 from Clarkson College. She has a variety of nursing experiences in post-anesthesia recovery, family practice and ENT. Her favorite part of her current role is the privilege of helping members make healthier lifestyles. Ann and her husband, Nate, live in Runnells, Iowa. They have three kids and spend their time with school and sports activities. Her favorite hobbies are reading, baking, gardening, being outdoors and traveling.

Vicki Giesselman, Nurse Practitioner

Vicki Giesselman is a board-certified advanced practice nurse practitioner with a degree in family medicine from Drake University, graduating in 2001. Before that, she was an ICU and ER nurse at Iowa Methodist in Des Moines for about 10 years. Vicki has practiced in a wide variety of settings, including family practice, emergency medicine and dermatology. She has two grown daughters and one grandson. Vicki and her husband David keep busy at home with their labradoodle and spoiling their grandson! They also enjoy motorcycling, fishing, boating and spending time at the family home on Big Spirit Lake. Vicki is so happy to be here at Vermeer! Her heart has always been in family practice, and she is thrilled to be practicing in a setting that allows her to spend adequate time with members, placing a focus not on productivity but on quality. 

Katrina Heater, RN, Care Management

 

Katrina Heater is a nurse and care management nurse with Premise Health at Vermeer Corporation. She obtained her RN degree at Indian Hills. Katrina has also obtained many certificates throughout the years, the latest being a certificate in wellness health coaching. She has a variety of healthcare experience, including emergency room, psychiatric, infection prevention, long-term care and skilled therapy. She has worked hands-on with patients and healthcare management as both an assistant and director of nursing. Katrina loves working for the Vermeer Clinic by meeting one-on-one with members and walking with them on their health journey as they meet their health goals and become the healthiest version of themselves. She also loves assisting Vermeer employees and their family members to find a resolution when unwell. In addition to care management, Katrina can be found in the lab and at flu clinics and biometric screening events. Katrina and her husband live locally in Pella with their four kids. They like to stay busy by being outdoors and attending their kids’ school activities.

Kina Wood, Medical Assistant and Phlebotomist

Kina Wood is a dually-certified medical assistant and phlebotomist, working in primary care at Vermeer since 2017. In addition to primary care, she also performs audiometry hearing exams for Vermeer team members as part of occupational health services. Kina resides in Pella and enjoys spending time with her two adult daughters and their families. She studied nursing at Marshalltown Community College and has acquired a wide range of experience in both clinical and hospital settings throughout her career. What Kina loves most about working at Vermeer Health Services are the relationships she’s built, in addition to fulfilling her interest in diagnostics, lab and helping people navigate healthcare.

Mickie Templeton, NCMA

Mickie Templeton is a national certified medical assistant (NCMA) and receptionist at Premise Health. Prior to joining Premise Health, she had experience working in the factory setting, obstetrics/gynecology, and primary care health. Mickie received her NCMA certification at Hamilton Tech College in Davenport, IA in 2008 and has been onsite at Vermeer since 2018. She enjoys getting to know the members and their families. Mickie takes pride in her work with making each person feel welcomed. In her spare time, she adores spending time with her two beautiful daughters and attending their activities, as well as bonding with her family and friends. She also enjoys reading, bonfires and the outdoors.

Matt Doty, Medical Director, OCC Health Medical Director

 

Matt Doty is a lifelong Iowa native, having grown up in Grinnell, IA before completing undergraduate study at Iowa State University in 1992 and medical school at the University of Iowa in 1998. He has been employed at Pella Regional Health Center since 2001, working as an emergency room physician until 2013 before making a transition to occupational health. Matt enjoys the diversity an occupational medicine position provides and has a special interest in the treatment of work injuries. He also serves as medical review officer and professional supervisor of the audiometric monitoring program. 

Matt has been medical director for Vermeer occupational health since 2013 and am proud to serve in this role due to Vermeer’s commitment to the health and wellness of their employees. In particular, Vermeer’s philosophy of providing appropriate and timely care to injured workers, as well as having an onsite medical center and pharmacy, are examples of this commitment.

Matt has resided in Pella, IA since 2001 and enjoys spending time with his wife Kim and children Pearl, Camille and Sam. He is an avid golfer and enjoys reading and attending sporting events.

Leah Bruxvoort, Nurse Practitioner, OCC Health

 

Leah Bruxvoort is a nurse practitioner in occupational health with Premise at Vermeer Corporation. She has been with Premise Health since 2017 when she initially worked in primary care, moving to occupational health in 2022. She graduated with her nursing degree in 2005 from St. Luke’s College of Nursing, BSN degree from University of Iowa and in 2012 with an MSN-FNP degree from Clarkson College in Omaha, NE. Leah has had a variety of nursing experiences in labor/delivery, med/surg, high-risk obstetrics, family practice and occupational health. Her favorite part of her current role is the privilege to listen to daily struggles at work and home life. She wants occupational health to be a place where members can be heard and cared for physically and mentally. Leah and her husband Rob live outside of New Sharon, Iowa where they raise hogs and grow corn and beans. They have five kids and spend their time with 4-H projects, church and school activities. Leah’s favorite hobbies are hiking, camping and being outside in the sunshine.

Gina Jones, RN, OCC Health

 

Gina Jones has 16 years of experience in clinical nursing and nursing leadership. She graduated from Grand View University in 2007 with her Bachelor of Science in nursing (BSN) degree. Gina also maintains a specialty certification as a certified emergency nurse (CEN). Her experience includes emergency and critical care nursing, most recently at UnityPoint Health – Des Moines. Gina takes pride in being able to provide frontline care to the injured member. She enjoys the partnership with Vermeer while serving as an advocate for the Vermeer team members. In her free time, Gina enjoys running, fishing and watching her kids in their sporting events.

Amanda Lathrum, RN, OCC Health

Amanda Lathrum is a registered nurse case manager for Vermeer. She graduated from Hesston College located in Kansas with an associate degree in nursing. Amanda’s healthcare career began in the emergency department at Wesley Medical Center in Wichita. The opportunity arose to pursue travel nursing which led her to Kansas City and Iowa. After permanently moving to Iowa, she completed her final nursing contract at The Newton Correctional Facility. Her array of nursing experience, primarily being ER, has contributed to her growth and development as a nurse. Through Premise’s partnership with Vermeer, Amanda is excited to be able to provide team members with access to onsite medical care. She resides locally along with her husband, three children and two Goldendoodles. Amanda has an avid love for DIY projects and spends most of her free time attending sporting events.

Barbara Ebert, Administrative Assistant, OCC Health

 

Barbara Ebert works as an administrative assistant and manage the pre-hire process for occupational health. Her background is in caregiving and business office management. Barbara enjoys learning new things and is never afraid to jump in and help wherever she is needed. Caring for people has been her lifelong passion. She enjoys gardening, sewing, spending time with her grandchildren and running her upholstery business on the side. Barbara is happy to play a part in the wonderful partnership between Premise Health and Vermeer.

Tom Naeve, Pharmacy Manager

Tom is the pharmacy manager at the Pharmacy @ The Center. Tom is a Drake University College of Pharmacy graduate. He has been a registered pharmacist since 1999 and onsite at Vermeer since 2008.

Shannon Arkema, Senior Pharmacy Technician

Shannon has a wealth of pharmacy experience spanning over 20 years. She has covered about every aspect of pharmacy including retail, non-sterile and sterile compounding, home health and home infusion. Shannon welcomes the opportunity to help you best manage your health and wellbeing.

Premise Health is the organization that operates and staffs your wellness center[s]. We are the nation’s leading provider of direct healthcare, which means we partner with companies to offer their employees high-quality in-person and virtual health services. We are here as your health ally to help you get, stay and be well. We do this with experience, and by shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

Members are what we call our patients. If your employer has partnered with Premise Health to provide access to our wellness centers and services, you’re automatically considered an eligible member. There is no membership fee to get started. It also reflects how much we value our care relationship with you, because to us, you’re more than just a patient – you’re a member.

Wellness @ The Center is staffed, managed and operated by Premise Health. The medical staff includes a combination of physicians, nurse practitioners, care management registered nurses and medical assistants. Each staff member has been carefully screened and is qualified to deliver exceptional care. Your wellness center is not staffed or managed by Vermeer or their employees in any way. 

My Premise Health is your member portal. It is a secure online and mobile tool that you can use to manage your health information. My Premise Health connects you to your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

Wellness @ The Center is available to all employees eligible for Vermeer’s insurance plan and their eligible dependents up to age 26. 

Services are free to team members who are enrolled in non-HDHP health coverage. If enrolled in HDHP health coverage, a nominal $40 fee is payroll deducted. 

Confidentiality

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the Vermeer Family Pharmacy & Health Center. 

Yes, you may request any records be sent to another medical provider. If your community provider uses Epic (or My Chart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa. 

Scheduling

You can easily schedule appointments through the My Premise Health mobile app or online at mypremisehealth.com. Once you create your account, sign in and select “Schedule an Appointment” and choose your appointment type.

In most cases, same-day appointments are available. Schedule online or call your wellness center to see if a same-day appointment is available.

While walk-ins are gladly accepted, the preferred method is to schedule an appointment ahead of time. Same-day appointments are typically available.

Virtual visits

To meet virtually with one of the providers at your wellness center, you must first create an account on the My Premise Health app or online at mypremisehealth.com. Log in and select “Schedule an appointment,” choose your appointment type and if prompted, select whether you’d like to be seen by phone or video. You can then follow the remaining prompts to complete your scheduling.

A virtual visit is just like an in-person appointment. You can discuss your medical needs with your provider, and they will diagnose and prescribe medications as needed. 

Virtual visits are designed to treat your short-term needs and provide long-term preventive care with convenience and flexibility. If you’re experiencing a medical emergency, please dial 911 as soon as possible to get the medical attention you need. 

Services

Preventive care includes routine checkups and screenings that help you stay healthy in the long-term. These are visits you make when you’re feeling well to catch any health issues early or improve your wellbeing. This includes annual physicals, cancer screenings, vaccinations and more.

Non-preventive care, on the other hand, includes visits you schedule when you’re not feeling well. These are meant to address your symptoms to find out what’s wrong and get you back to feeling your best. Some examples include sick visits and treatment for issues like cold and flu, allergies, injuries and more.

Yes, your wellness center can perform blood draws and other lab tests ordered by an outside provider. All lab orders must be faxed or dropped off at the center to ensure we have the proper supplies for the required testing.

You can call the pharmacy at (641) 628-3738 to speak with a pharmacist.

You can easily drop off your prescriptions at Pharmacy @ The Center or your provider can e-prescribe them directly to the pharmacy. The providers and pharmacy at your wellness center act as one care team. Your provider will send your prescription to the pharmacist after your appointment so you can pick it up the same day, making for a seamless experience.
