Jeffrey Nelson, DO, FACOFP, AAHIVS, Medical Director
Dr. Nelson graduated with a Doctor of Osteopathic Medicine (DO) degree from Western University of Health Sciences and completed a family medicine residency from Advent Health East Orlando. He is also an HIV medicine specialist, certified through the American Academy of HIV Medicine.
In his free time, Dr. Nelson volunteers at the non-profit organization, Shepherd’s Hope. He also loves traveling with his wife and daughter, visiting theme parks, and watching football.
Kate Reyes, MS, FNP-C, Nurse Practitioner
Kate Reyes is a Family Nurse Practitioner certified by the American Academy of Nurse Practitioners. She received her Bachelor of Arts in Spanish from the University of Hawai’i, her Bachelor of Science in Nursing from the University of North Florida, and her Master of Science (FNP) from Georgetown University. She is now pursuing her Doctorate of Nursing Practice at the University of Central Florida.
Prior to serving as a family primary care provider in Orlando, Kate worked as a hospital RN for 5 years in Washington, D.C. She is passionate about preventative care and helping patients to feel their best at all stages of life and is also fluent in Spanish.
In her free time, Kate volunteers at the non-profit organization, Shepherd’s Hope, goes on yearly medical mission trips, gardens, plays with her dog, and studies! Her favorite thing is to go on adventures with her family and friends.
Tina Tolentino, MSN, RN, Group Health Center Manager
Tina began her healthcare career as a Certified Medical Assistant and furthered her career in 2016, earning her Nursing degree and Registered Nurse license. She recently went back to school and received her Master of Science in Nursing degree with Chamberlain University. She is a Florida native and an active member of the Florida Nurses Association, and Sigma Theta Tau International Honor Society of Nursing.
Tina has worked as a Nurse and leader since 2017. Her favorite part of her role are the relationships she gets to build with her patients over time, being there to help them in the hard times, and watching them enjoy their health and wellness.
In her free time, Tina loves spending time with her family, cruising, and enjoying the Central Florida attractions!
