Primary care

We offer a comprehensive range of primary care services that focus on your long-term health, including sick and preventive care and management of your chronic conditions. Your primary care provider will get to know you so they are better equipped to prevent, treat and manage any health issues that may come up. The best part? Care management

You don’t have to navigate your health challenges alone. With our care management services, you can get the support you need. This no-cost benefit provides a registered nurse as your care manager to be a go-to resource for all health-related questions, prescription guidance and more. They will come up with personalized treatment plans to help you make positive lifestyle changes to prevent, improve, manage or even reverse certain health issues.

Our treatment plans are tailored to meet your unique needs and can address a range of conditions, such as asthma, high blood pressure, obesity, diabetes and pre-diabetes, low back pain and more. We can provide one-on-one coaching, disease-specific education, self-care training, nutrition counseling, medication guidance and ongoing monitoring to manage your symptoms and help you stay well.

Occupational health

Keeping you safe at work is a top priority. We offer a full range of occupational health services to treat work-related injuries and illnesses and help you get back to feeling well.

 

Our occupational health services include injury diagnosis and treatment, emergency care services, musculoskeletal evaluations, ergonomic assessments, pre/post-employment exams, drug and alcohol testing, physical therapy, occupational therapy services, and return-to-work plans.

Women's health

We understand a woman’s healthcare needs are unique. Your provider will spend time getting to know you to best understand your health goals. They’ll come up with a personalized care plan to support your long-term health. 

Along with general primary care services, we offer the following specifically for women’s health:

  • Breast cancer screenings
  • Pelvic exams
  • Cervical cancer screenings (Pap smears)
  • Pregnancy testing
  • Comprehensive family care
  • Contraception (birth control) counseling
  • STD testing and counseling
  • LGBTQ+ care
  • Hormone therapy
  • Education for self-awareness

Lab services

We offer many lab services to support your needs. We can perform onsite testing and can also collect and send samples to an offsite lab. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health account or during an in-person or virtual follow-up appointment with your provider.

Vaccines

Vaccines are an essential part of public health. You have convenient access to many vaccines at our health centers. Your provider can determine which ones are right for you based on your age, conditions, travel habits and other factors.


We offer a variety of routine vaccines to protect your health and community, including:

  • Influenza (Flu)
  • COVID-19
  • Hepatitis A
  • Hepatitis B
  • Human Papilloma Virus (HPV)
  • Pneumococcal (Pneumonia, Meningitis, etc.)
  • Tdap/Td (Tetanus, Diphtheria and Pertussis / Whooping Cough)
  • Zoster (Shingles)

Travel medicine

Your health doesn’t take a vacation. We offer travel medicine services to help you plan for your next trip and stay well while you’re away. Depending on your destination, we can advise on any vaccinations you may need. You can also talk to your provider about other possible health risks based on your history or conditions, and we can prescribe medications and make sure you have an ample supply so you won’t run out.


Our travel medicine services include pre-travel risk assessment, prescriptions and vaccinations based on your destination, risk reduction planning and education for self-treatment, and post-travel assessments and treatment.

Wellness

Our wellness programs promote nutrition, physical activity, stress management, healthy relationships and more. We take a comprehensive approach that focuses on your overall health and wellbeing. Our providers will personalize your wellness plan and help you make positive lifestyle changes that can lead to a more balanced and healthier you.


Your wellness services include biometric screenings, coaching and condition management, which may include weight and stress management, nutrition counseling, physical activity routines, tobacco cessation, work/life balance and life satisfaction.

My Premise Health

Our secure member portal makes it easy to connect with your care team. Whether you’re at home or on the go, you can schedule appointments, message your providers, view your lab results and more – anytime, anywhere.

You can schedule a virtual appointment with your trusted care team during our regular business hours, at the same cost as an in-person visit.

Frequently Asked Questions

Premise Health is the organization that operates and staffs your wellness center. We are the nation’s leading provider of direct healthcare, which means we partner with companies to offer their employees high-quality in-person and virtual health services. We are here as your health ally to help you get, stay and be well. We do this with experience, and by shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

Members are what we call our patients. If your employer has partnered with Premise Health to provide access to our wellness centers and services, you’re automatically considered an eligible member. There is no membership fee to get started. It also reflects how much we value our care relationship with you, because to us, you’re more than just a patient – you’re a member.

Your wellness center is staffed, managed and operated by Premise Health. The medical staff includes a combination of nurse practioners. Each staff member has been carefully screened and is qualified to deliver exceptional care. Your wellness center is not staffed or managed by your employer in any way.

Absolutely. If you are a JMS employee, you are eligible to use the health center, regardless of insurance coverage. Please talk to the nurse practitioner for further details if you have outside health insurance or have elected to have no coverage.

All preventive, nurse services and occupational visits are at no out-of-pocket costs to you. Acute care visits, such and illness or personal injury are $35 and you may use your HSA card to pay for services. Any labs that are required, beyond preventive will be billed to you directly from Quest laboratories, based on your insurance coverage.

 

If you are a JMS employee, you are eligible to use the health center, regardless of insurance coverage. Please talk to the nurse practitioner for futher details if you have outside health insurance or have elected to have no coverage.

 

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.

The easiest way to make an appointment is through your online member portal. Visit mypremisehealth.com to set up your account and then schedule appointments online or through your My Premise Health mobile app.

While walk-ins are gladly accepted, the preferred method is to schedule an appointment ahead of time.
Same-day appointments are typically available.
