Premise Health is a provider of in-person and virtual health for the world’s leading organizations. In other words, we partner with companies to bring their employees healthcare services and operate their wellness centers.

Members are what we call our patients who are eligible to use a wellness center operated by Premise Health. It’s not a membership that you have to pay for or apply to be a part of. You’re simply considered a member once your employer has partnered with us to provide your health and wellness services.

A wellness center is where you receive care as a member. We use this term versus the typical “doctor’s office” because we offer so much more than the usual office environment. Our centers not only typically offer more services than the average doctor’s office, but are also designed with you in mind, so you feel comfortable, calm and welcomed.

Bachoco OK Foods Premise Health Wellness Center is staffed, managed and operated by Premise Health. Each staff member has been carefully screened and is qualified to deliver exceptional care. Bachoco OK Foods Premise Health Wellness Center is not staffed or managed by Bachoco OK Foods or their employees in any way.

Arkansas and Oklahoma employees and their family members seeking care at the wellness center must be covered under one of Bachoco OK Foods medical plans to use the center. You and covered family members can begin using the center starting on the effective date of your medical plan.

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. The easiest way to make an appointment is through your online member portal. Visit mypremisehealth.com to set up your account and then schedule appointments online or through your My Premise Health mobile app.

You can also call (479) 974-1270 or visit during regular center hours to make an appointment. Same-day appointments are typically available.
