Dr. Johann Serag, MD – Medical Director
Dr. Johann Serag is the medical director at the Premise Health Wellness Center in Northbrook, IL. He brings over a decade of experience in the field of Family Medicine. He is particularly interested in musculoskeletal problems, having an undergraduate degree in Physical Therapy. His passion lies in preventive medicine, treating a wide range of patients from kids to the more mature members of the community.
Patient Care Philosophy:
Dr. Serag believes in the saying “Wherever the art of medicine is loved, there is love for humanity”. Genuine appreciation for the skills and dedication involved in healing is intertwined with a deep concern for the wellbeing of the people.
How I Stay Well:
Dr. Serag maintains his mental wellbeing by being involved in different arts and crafts, mainly collaborating with his 3 young kids, building miniaturized scale models and meditating. Dr. Serag also stays involved in seasonal sports such as golf, pickleball, basketball, hiking and biking.

Carla Johns, FNP-BC, MSN – Family Nurse Practitioner
Carla Johns is a Board-Certified Family Nurse Practitioner with over 25 years’ experience in healthcare. Carla graduated from the University of Iowa with a Bachelor of Science in Nursing. She worked as a registered nurse at both the inpatient and outpatient settings before going on to attain a Master’s in Nursing Science from University of Illinois Chicago in 2007. Since then, Carla has worked as a nurse practitioner in primary care and has 13 years with Premise health.
Patient Care Philosophy
Carla practices with shared decision making in mind, valuing patients’ input in their care. She has vast experience in weight management, tailoring recommendations for each person’s needs. She has also been successful in improving outcomes for those with chronic illness and reducing medication burden.
How I Stay Well:
Carla practices what she preaches. She hits her exercise goals through both Orange Theory and CrossFit classes. When not formally exercising, Carla walks with her dog or practices sports with her kids, who are active in volleyball and softball. She values the importance of social connections and makes time to see friends and family each week.

Camilla Philippe, LCSW – Licensed Clinical Social Worker
Camilla received her Bachelor’s in Social Work from Western Illinois University and completed her Master’s in Social Work at Aurora University. She has worked extensively in a multitude of settings including mental health facilities, nursing homes, and homeless shelters. Camilla has been in social work for about 10 years.
Camilla has experience working with children, adolescents, adults, families, and couples. She specializes in anxiety, depression, stress management, relationship concerns, low self-esteem, and chronic mental illness. Camilla uses Solution Focused treatment, Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT), and Strengths based approaches as my treatment approaches.
Patient Care Philosophy:
She is compassionate, non-judgmental, and an empathic listener. She believes in personal growth and a collaborative therapeutic relationship to create treatment plans to assist and support her clients in achieving their goals.

Dr. Sultan Budhwani, PT, DPT – Physical Therapist
Board Certified Orthopedic Clinical Specialist
Dr. Budhwani is a physical therapist and Board Certified Orthopedic Clinical Specialist, dedicated not only to helping clients regain their independence, but maintaining it. Sultan has a strong background in strength and conditioning as a Personal Trainer prior to completing his Physical Therapist degree. His practice style consists of therapeutic exercises, neuromuscular rehabilitation, joint mobilizations/manipulations, soft tissue mobilizations and trigger point dry needling. He has nearly 10 years of clinical instruction experience with physical therapy students and has taught at the University of Illinois in Chicago for their Doctor of Physical Therapy Program.
Patient Care Philosophy:
There are two major components to his method of treatment. Number 1 is independence. Sultan believes that everyone should be able to achieve and/or maintain their independence. Number 2 is stimulus for change. He believes change can only be achieved with proper stimulation and progressive overload. That change will then facilitate progress towards a client’s goals and maintain their independence.
How I Stay Well:
When not practicing, Dr. Budhwani enjoys regular strengthening and conditioning mixed between body building and power lifting, spending time with family, and pursuing continuing education opportunities in the area of physical medicine and rehabilitation. Additional interests include woodworking, teaching/education, and volunteering at a local mosque.

Dr. Jose Aguilar, DC – Chiropractic Physician
After watching his mother go from debilitating migraines that severely affected her ability to care for her family to having them completely resolved due to chiropractic care, he found his calling. Born and raised in the Quad Cities, Dr. Jose Aguilar earned his Doctorate of Chiropractic from Palmer College of Chiropractic in 2004, after earning two degrees in Health Sciences and Liberal Studies from Black Hawk College. During his time at Palmer, he and 25 others spent 3 weeks on a missionary trip to Fiji providing chiropractic care to underserved communities. After graduating, Dr. Aguilar also earned his Certification as a Strength and Conditioning Specialist and an additional Certification in Therapeutic Taping.
Dr. Aguilar has also done various lectures with numerous organizations, speaking on a variety of topics including Stress, Nutrition, Peak Performance, Proper Posture, Injury Prevention, Whiplash and more. He was also recognized as a Top Recommended Professional as a Chiropractor and has been quoted on NBC, CBS, ABC and FOX, and has been inducted into the National Academy of Best-Selling Authors as an award-winning author for co-authoring a book with Brian Tracy and other contributors.

Dr. Aguilar frequently attends post-graduate seminars at some of the most prestigious venues such Harvard School of Medicine, Spine Research Institute of San Diego among others, to stay on top of a wide variety of topics and implements the most up-to date information available in order to help his patients succeed in reaching their specific health goals.

How I Stay Well:
Dr. Aguilar maintains his own health and well-being with regular exercise and eating healthy foods, especially his wife’s cooking. A devoted father to his children, Isabel and Orlando, Dr. Aguilar also enjoys traveling abroad and playing sports during his leisure hours and is fluent in Spanish.
