Mental health

We believe that your mental and emotional wellbeing are just as important as your physical health. With our mental health services, you can get the support you need. Your licensed counselor will actively work as part of your care team to support your overall health while making you feel safe and comfortable. You can conveniently meet with them in person or virtually, through video or by phone.

With a blended approach of counseling techniques and evidence-based talk therapy, our counseling services cover anxiety, depression, stress management, PTSD, ADHD, grief, adjustment challenges, substance abuse, work/life balance, family conflict and sleep issues.

Provider-dispensed medications

Skip the extra trip to the pharmacy. You can get your medications at little to no cost directly from your provider during a visit. Simply schedule an appointment, discuss your health needs and leave with your medication in hand.

Our health centers carry close to 100 medications to cover your needs. This includes over-the-counter options. You can even get up to a 90-day supply for ongoing prescriptions to reduce the number of times you need a refill. When you’re about to run out, schedule a follow-up visit with your provider. They’ll monitor your progress and offer ongoing support.

Women's health

We understand a woman’s healthcare needs are unique. Your provider will spend time getting to know you to best understand your health goals. They’ll come up with a personalized care plan to support your long-term health. Along with general primary care services, we offer the following specifically for women’s health:

Chiropractic care

Lab services

We offer many lab services to support your needs. We can perform onsite testing and can also collect and send samples to an offsite lab. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health account or during an in-person or virtual follow-up appointment with your provider.

We can perform and process the following onsite:

Vaccines

Vaccines are an essential part of public health. You have convenient access to many vaccines at our health centers. Your provider can determine which ones are right for you based on your age, conditions, travel habits and other factors.

We offer a variety of routine vaccines to protect your health and community, including:

Care Management

Care management offers guided support when you need it most. Whether you’re juggling multiple conditions or just need a little extra support getting your health in check, your care manager is here to help.

You have a dedicated care manager available through your wellness center. As a licensed nurse, they’ll work with your care team to provide [personalized plans, coaching, education and ongoing monitoring and follow-ups] throughout your care journey. Through hands-on guidance and regular check-ins, they can help with a range of health concerns and conditions, including [diabetes, cancer, Lupus, hypertension, mental health issues or any other health challenge or condition that may require a need for frequent touchpoints with your care team].

Finding that care on your own can sometimes be confusing. That’s why we provide care navigation to support you in finding the affordable, high-quality care you need.

If your provider gives you a specialist referral, your care navigator will help you find the best care at the best price in your community. They’ll even assist with scheduling your appointments, transferring your medical records and following up after your specialist visit to help with any ongoing care needs.

Care Consult

If you receive an abnormal test result, are told by a provider in your community that you need major surgery or experience some other serious health issue, you may benefit from a second opinion. Your providers at [Wellness Center Name] can offer that through Premise Health’s partnership with Mayo Clinic. They’re able to virtually consult with specialists at Mayo to review your medical record and come up with a recommended treatment plan for your specific health concern. Your provider will then share the outcome of that conversation with you to help you navigate your care plan and any next steps.

Schedule a visit with your provider to get started. Should you need a second opinion from a specialist, your provider will facilitate that through care consult.

Care management

You don’t have to navigate your health challenges alone. With our care management services, you can get the support you need. This no-cost benefit provides a registered nurse as your care manager to be a go-to resource for all health-related questions, prescription guidance and more. They will come up with personalized treatment plans to help you make positive lifestyle changes to prevent, improve, manage or even reverse certain health issues.

Our treatment plans are tailored to meet your unique needs and can address a range of conditions, such as asthma, high blood pressure, weight control, diabetes and pre-diabetes, low back pain and more. We can provide one-on-one coaching, disease-specific education, self-care training, nutrition counseling, medication guidance and ongoing monitoring to manage your symptoms and help you stay well.
