Primary care

We offer a comprehensive range of primary care services that focus on your long-term health, including sick and preventive care and management of your conditions. Your primary care provider will get to know you so they are better equipped to prevent, treat and manage any health issues that may come up. 

Our lifestyle medicine approach ensures we’re looking at the whole picture of what impacts your wellbeing, including sleep, nutrition, stress, movement and more. We can encourage healthy habits to help you achieve sustainable wellness and go beyond simply treating the symptoms you bring with you to a single appointment.

Care management

You don’t have to navigate your health challenges alone. With our care management services, you can get the support you need. This no-cost benefit provides a registered nurse as your care manager to be a go-to resource for all health-related questions, prescription guidance and more. They will come up with a personalized treatment plan to help you make positive lifestyle changes to prevent, improve, manage or even reverse certain health issues. 

Our treatment plans are tailored to meet your unique needs and can address a range of conditions, such as asthma, high blood pressure, diabetes and pre-diabetes, low back pain and more. We can provide one-on-one coaching, disease-specific education, self-care training, nutrition counseling, medication guidance and ongoing monitoring to manage your symptoms and help you stay well.

Women's health

We understand a woman’s healthcare needs are unique. Your provider will spend time getting to know you to best understand your health goals. They’ll come up with a personalized care plan to support your long-term health. 

Along with general primary care services, we offer the following specifically for women’s health:

  • Breast cancer screenings
  • Pelvic exams
  • Cervical cancer screenings (Pap smears)
  • Pregnancy testing
  • Comprehensive family care
  • Contraception (birth control) counseling
  • STD testing and counseling
  • LGBTQ+ care
  • Education for self-awareness

Lab services

We offer many lab services to support your needs. We can perform onsite testing and can also collect and send samples to an offsite lab. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health account or during an in-person or virtual follow-up appointment with your provider.

Vaccines

Vaccines are an essential part of public health. You have convenient access to many vaccines at the Hearst Wellness Center. Your provider can determine which ones are right for you based on your age, conditions, travel habits and other factors.

We offer a variety of routine vaccines to protect your health and community, including:
  • Influenza (Flu)
  • COVID-19
  • Hepatitis A
  • Hepatitis B
  • Human Papilloma Virus (HPV)
  • Measles, Mumps and Rubella (MMR)
  • Pneumococcal (Pneumonia, Meningitis, etc.)
  • Tdap/Td (Tetanus, Diphtheria and Pertussis / Whooping Cough)
  • Varicella (Chickenpox)
  • Zoster (Shingles)

Wellness

The Hearst Wellness Center takes a holistic approach to your health by focusing on your overall wellness, preventive care and chronic conditions. Our wellness programs promote nutrition and physical activity, as well as managing stress and relationships. Your wellness providers will personalize your wellness plan and help you make positive lifestyle changes for a healthier you.

Your wellness services include biometric screenings, wellness coaching and condition management.

Travel medicine

At the Hearst Wellness Center, we offer travel medicine services to help you plan for your trip and to stay well while you’re away. Based on your destination, things like getting vaccinated, refilling your prescriptions and discussing health risks with your provider can make or break your travel experience.

Our travel medicine services include pre-travel risk assessment, prescriptions and vaccinations based on destination, risk reduction planning and education for self-treatment, as well as post-travel assessments and treatment or specialist referral.

You can schedule a virtual appointment with your trusted care team during our regular business hours, at the same cost as an in-person visit.

Meet your care team.

Calista Yong, Certified Physician Associate

Calista Yong is a certified physician associate with a Bachelor of Science degree in physician assistant studies from St. John’s University. She has been working as a physician associate for over 10 years and is passionate about educating people and empowering them to take control of their health. One of the most rewarding parts of her work is being able to build strong relationships, listen to members’ concerns and guide them through their healthcare journey. When she isn’t working, Calista enjoys exploring the world through hiking and traveling.

Jeanine Troisi, RN

Jeanine is a BSN registered nurse with over 15 years of experience in various fields of nursing, including hospice, primary care, occupational health, oncology, infectious diseases and medical/surgical. She holds licenses in New Jersey and New York, and is CPR/BLS, PRI and SCREEN certified.

For the past five years, Jeanine has served as an RN field case manager in hospice for VNS Health and MJHS, collaborating with and leading an interdisciplinary team to manage patients in palliative care. She has previously worked as a triage/occupational health registered nurse at West Midtown Medical Group, primary care facility where she managed the employee health program and achieved 97% compliance with the flu shot program over two years. Jeanine has previously worked in oncology at Mount Sinai West, in primary care at Callen-Lorde Medical Center, serving the needs of the LGBT community, and on a medical/surgical unit at RWJ Barnabas Health systems in Livingston, NJ.

She has excellent leadership, critical thinking and communication skills and is proficient in several electronic health record systems.

She holds a Bachelor of Science degree in nursing from Drexel University, where she was inducted into the Sigma Theta Tau International Nursing Honor Society and Delta Epsilon Iota Honor Society, and a Bachelor of Arts degree in communications from William Paterson University. She is passionate about providing quality care, enhancing her skills and knowledge as a nurse and providing efficiency, support and structure as a manager.

Members are what we call our patients. If your employer has partnered with Premise Health to provide access to our wellness centers and services, you’re automatically considered an eligible member. There is no membership fee to get started. It also reflects how much we value our care relationship with you, because to us, you’re more than just a patient – you’re a member.

The Hearst Wellness Center is staffed, managed and operated by Premise Health. The medical staff includes a combination of care management registered nurses and medical assistants. Each staff member has been carefully screened and is qualified to deliver exceptional care. Your wellness center is not staffed or managed by Hearst or their colleagues in any way. 

My Premise Health is your member portal. It is a secure online and mobile tool that you can use to manage your health information. My Premise Health connects you to your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

Eligibility and cost

All Hearst colleagues who are enrolled in or eligible for UnitedHealthcare or Cigna medical coverage and work in the Hearst Tower or Sheffield can use primary care services offered at the Hearst Wellness Center. As always, all other employees can access acute care at the Hearst Wellness Center, including immunizations and vaccinations and treatment for common conditions such as colds, flu, infections, allergies and more, but they are not eligible for primary care services.

All Hearst colleagues who are enrolled in or eligible for UnitedHealthcare or Cigna medical coverage and work in the Hearst Tower or Sheffield can use primary care services offered at the Hearst Wellness Center. As always, all other employees can access acute care at the Hearst Wellness Center, including immunizations and vaccinations and treatment for common conditions such as colds, flu, infections, allergies and more, but they are not eligible for primary care services.

  • If you are enrolled in the UnitedHealthcare Consumer Plan or either of the Choice Fund Open Access Plus HSA Plans (HDHP 1600 or HDHP 3000), a primary care visit costs $35 toward your deductible. Once you reach your deductible, visits are free.
  • If you are enrolled in the UnitedHealthcare Classic Plan or the Open Access Plus Plan (OAP 600), a primary care visit costs $10 per visit.
  • If you are eligible for medical coverage through UnitedHealthcare or Cigna but waived it, primary care visits cost $50.

Please note that preventive care, such as annual physicals and screenings, is always provided at no cost to you and with no deductible when you use an in-network provider, such as the Hearst Wellness Center, and are enrolled in UnitedHealthcare or Cigna medical coverage.

Confidentiality

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.

Yes, you may request any records be sent to another medical provider. If your community provider uses Epic (or My Chart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.

Scheduling

You can easily schedule appointments through the My Premise Health mobile app or online at mypremisehealth.com. Once you create your account, sign in and select “Schedule an Appointment” and choose your appointment type.

In most cases, same-day appointments are available. Schedule online or call your wellness center to see if a same-day appointment is available.

While walk-ins are gladly accepted, the preferred method is to schedule an appointment ahead of time. Same-day appointments are typically available.

Virtual visits

To meet virtually with one of the providers at your wellness center, you must first create an account on the My Premise Health app or online at mypremisehealth.com. Log in and select “Schedule an appointment,” choose your appointment type and if prompted, select whether you’d like to be seen by phone or video. You can then follow the remaining prompts to complete your scheduling.

A virtual visit is just like an in-person appointment. You can discuss your medical needs with your provider, and they will diagnose and prescribe medications as needed. 

Virtual visits are designed to treat your short-term needs and provide long-term preventive care with convenience and flexibility. If you’re experiencing a medical emergency, please dial 911 as soon as possible to get the medical attention you need.

Yes, your wellness center can perform blood draws and other lab tests ordered by an outside provider. All lab orders must be faxed or dropped off at the center to ensure we have the proper supplies for the required testing.
