Provider Visits $40 Flat Fee*

This includes primary care visits for services such as:

  • Cold, flu or allergy symptoms
  • Nausea, vomiting or diarrhea
  • Muscle or joint pain
  • Rashes, insect bites and stings
  • Headache, earache, etc.
  • Testing: flu, strep, Covid-19
  • Urinalysis and urine pregnancy

Physical Therapy

$45 flat fee*

  • Physical therapy visits $45 flat fee

    • Shoulder, neck and back pain
    • Muscle/joint pain or impairment
    • Sprains and strains
    • Pre-and post-surgical conditions
    • Headaches (tension-related)
    • Carpal tunnel and sciatica

You can request a phone or video visit with the providers at The Fiserv Wellness Center – Alpharetta through the My Premise Health app or online at mypremisehealth.com. This convenient option gives you the flexibility to schedule a virtual visit during our regular business hours, at the same cost as an in-person visit.

Meet your care team.

Kelly Perdomo, FNP-C

Kelly has been a nurse practitioner for 7 years. Prior to becoming a nurse practitioner, Kelly worked as a registered nurse at a Cardio-Thoracic Step-Down unit and an operating room for over 4 years. Kelly has a wide array of clinical experience; however, her focus has been Family Practice and Urgent Care.

 

Education: Kelly obtained her BSN from Clayton State University and her MSN from Chamberlain University. She is a member of The Sigma Theta Tau International Honor Society of Nursing.

 

Fun fact: Kelly enjoys volunteering and participating in community activities.

Emily Tretinik, DPT Physical Therapist

Emily was called to physical therapy in the second grade at an elementary career day event. She followed her career goals and continues to find joy in helping others. She has a background in brain injury rehabilitation from Prisma Health as well as orthopedic rehabilitation from Emory Healthcare.

Education: Emily graduated cum laude from The University of Georgia and completed her post-graduate studies at Armstrong Atlantic University in consortium with the Medical College of Georgia. She obtained her Doctorate of Physical Therapy degree in 2010.

Emily finds value in listening, learning, and working together to accomplish goals and maximize outcomes.

Martina Marquez Trimiew, BA, BSN, RN

Condition Management RN/Coach – Alpharetta 

 

Martina has been a Registered Nurse for nearly a decade. Her experience includes cardiovascular surgery, general cardiology, allergy/immunology, and occupational health. She is passionate about holistic nursing and preventative medicine. 

 

Education: Martina obtained her BA in Spanish from Virginia Commonwealth University and her BSN from Drexel University. She obtained her Holistic Nurse Coach certification from the Integrative Nurse Coach Academy. Martina is a trauma-informed yoga teacher who is also trained in breathwork, mindful meditation, and sound therapy. 

Andrea Corey, Referral Coordinator

Andrea, a native of Brazil who recently moved from California, is an experienced referral coordinator with a strong foundation in Mental Health, holding certifications in medical terminology, medical billing, and ICD-10 and ICD-11 coding. Andrea is passionate about community involvement, bringing a holistic approach to healthcare coordination.

 

Committed to facilitating seamless referrals and promoting health through collaborative community efforts. Additionally, she has dedicated five years as a volunteer membership secretary in local community, showcasing her commitment to service and organizational excellence. 

 

Fun fact, Andrea was a dedicated Girl Scout Leader, guiding young girls from Daisies to Juniors. Outside of work, Andrea cherishes family time and finds joy in biking, hiking, and exploring new destinations through travel.

Heather Evans, MA - Medical Assistant

Heather, a native Georgian, brings a decade of healthcare experience to her role at Premise Health. Her previous roles were in Women’s Health, geriatrics and memory care. She is deeply passionate about providing quality care to all she encounters. Outside of the clinic she is a mom of two girls, three fur babies and a bearded dragon! She loves being outdoors whether she is gardening or just going for a hike.
