Women's health

Women have unique healthcare needs. Our providers spend time getting to know you, or the women in your life, to best understand your health goals. They’ll come up with a personalized care plan to support your long-term health. Along with general primary care services, we offer the following specifically for women’s health:[breast cancer screenings, pregnancy testing, pelvic exams, contraception counseling, STD testing and counseling, cervical cancer screenings (Pap smears), counseling for interpersonal and domestic violence, LGBTQ+ care, comprehensive family care, hormone therapy and education for self-awareness. Women’s health services are available for all eligible City of Kenosha employees.

Condition management

You don’t have to navigate your health alone. With our condition management services, you and your eligible dependents can get the support you need to manage your conditions and take control of your health. Our providers will come up with personalized treatment strategies to help you make positive lifestyle changes to prevent, improve, manage or even reverse certain health issues.

Our treatment plans are tailored to meet your unique needs and can address a range of conditions, such as asthma, high blood pressure, diabetes and pre-diabetes, low back pain and more. We can provide [one-on-one coaching, disease specific education, self-care training and condition management tactics, diet guidelines and physical activity goals, medication counseling and preventive and ongoing monitoring to manage your conditions and help you and your family stay well.

Vaccines

Vaccines are an essential part of public health. You have convenient access to many vaccines at City of Kenosha Health and Wellness Clinic. Your provider can determine which ones are right for you and your eligible dependents based on your age, conditions, travel habits and other factors. You can then easily schedule an appointment or simply walk in to get your vaccines.

We offer a variety of routine vaccines to protect your health and our community, including Influenza, Hepatitis A, Hepatitis B, Human Papilloma Virus (HPV), MMR, Pneumococcal, Tdap/Td, Varicella and Zoster.

Lab services

To support you and your eligible dependent’s needs, we offer clinical lab services that include onsite testing and specimen collections for offsite processing. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health patient portal or during an in-person or virtual follow-up appointment with your provider.

We can perform and process the following lab tests onsite: glucose or lipid testing, hemoglobin A1C, rapid strep testing, influenza testing, urine pregnancy testing, urine dip screenings, rapid mono testing and H-pylori testing. Simply request a virtual visit to get started.

Wellness

The Health and Wellness Clinic takes a holistic approach to your health by focusing on your overall wellness, preventive care and chronic conditions. Our wellness programs promote nutrition and physical activity, as well as managing stress and relationships. Your wellness providers will personalize your wellness plan and help you and your eligible dependents make positive lifestyle changes for a healthier you.

Mental health

At the Health and Wellness Clinic we believe that your mental and emotional wellbeing are just as important as your physical health. With our mental health services, you and your eligible dependents can get the support you need. Our Nurse Practicioner actively works as part of your care team to support your overall health while making you feel safe and comfortable. You can conveniently reach them in-person or virtually, through video or by phone. *

With a blended approach of counseling techniques and evidence-based talk therapy, our mental health counseling services cover anxiety, depression, stress management, PTSD, ADHD, grief, adjustment challenges, substance abuse, work/life balance, family conflict and sleep issues. How to get started

You can talk to a provider by video or phone on the My Premise Health app or at mypremisehealth.com.

To schedule a virtual visit with a provider from your wellness center, log in to your My Premise Health account and select “Schedule an Appointment.” Choose your desired appointment type from the available “Telephonic” or “Video” options and complete the prompts.

If you need care right away, log in to My Premise Health, select “Get Care Now” and follow the remaining steps to request an on-demand visit.

Frequently Asked Questions

Premise Health is a provider of in-person and virtual health for the world’s leading organizations. In other words, we partner with companies to bring their employees healthcare services and operate their wellness centers.

Members are what we call our patients who are eligible to use a wellness center operated by Premise Health. It’s not a membership that you have to pay for or apply to be a part of. You’re simply considered a member once your employer has partnered with us to provide your health and wellness services.

A wellness center is where you receive care as a member. We use this term versus the typical “doctor’s office” because we offer so much more than the usual office environment. Our centers not only typically offer more services than the average doctor’s office, but are also designed with you in mind, so you feel comfortable, calm and welcomed.

The Health and Wellness Clinic is staffed, managed and operated by Premise Health. Your medical staff includes a combination of a nurse and nurse practitioner. Each staff member has been carefully screened and is qualified to deliver exceptional care. Health and Wellness Clinic is not staffed or managed by City of Kenosha or their Employees in any way.

To be eligible to use the clinic, employees, spouses and dependents will need to be enrolled in the City’s health plan.

Employees & retirees of City of Kenosha who participate in the City’s medical plans as well as spouses and dependents of employees ages 2 or older. To be eligible, all employees, spouses and dependents must have a health risk assessment completed. Children will not be required to complete a health risk assessment.

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. The easiest way to make an appointment is through your online member portal. Visit mypremisehealth.com to set up your account and then schedule appointments online or through your My Premise Health mobile app.

While walk-ins are gladly accepted, the preferred method is to schedule an appointment ahead of time.
Same-day appointments are typically available. Call clinic ahead to make an appointment.

My Premise Health is your member portal. It is a secure online and mobile tool that you can use to manage your health information. My Premise Health connects you to your health by providing convenient access to book and manage appointments, message your providers, conduct virtual visits, view visit history, see your test results and more.

Virtual primary care is a digital healthcare service through Premise Health. It offers management of your ongoing care needs as well as 24/7/365, on-demand access to a virtual care team from anywhere within the United States. You can even designate a primary care provider (PCP) if you prefer to see the same provider on an ongoing basis by phone or video. There are two ways to designate your virtual primary care provider (PCP) and schedule a visit:
1. Call (833) 930-4530 to speak with a member of our virtual primary care team for next steps.
2. Fill out the form at members.premisehealth.com/virtual-primary-care/ under “How to get started” to get in touch with our team and designate your virtual primary care provider.
Once you have a designated virtual PCP and would like to request a visit with them, give us a call at (833) 930-4530 and we’ll schedule your appointment.

If you have not done so already, first set up your account on the My Premise Health app or online at
mypremisehealth.com. Then, log in and select “Get Care Now.” A Premise Health team member will then put you in touch with your provider to begin your visit.

Once you designate a virtual primary care provider, you will be able to see them each time you schedule a virtual appointment. Simply give us a call at (833) 930-4530 to request a visit with your dedicated provider, and we’ll schedule your appointment. If you want to access care right away, you can use the on-demand feature in My Premise Health under “Get Care Now,” which will connect you to the first available provider.
