Women's health
We understand that each woman is unique and so are your healthcare needs, which is why we offer services specifically for women. Our providers will spend ample time getting to know you, so they can come up with a personalized care plan to support your long-term health. Along with our general primary care services, we offer breast cancer screenings, pregnancy testing, pelvic exams, contraception counseling, STD testing and counseling, pap smears, counseling for interpersonal and domestic violence, LGBTQ+ care, comprehensive family care, hormone therapy and education for self-awareness.
Condition management
You don’t have to navigate your health alone. With our condition management services, the City of Greeley Employee Wellness Center can get the support you need to manage your conditions and take control of your health. Our providers will come up with personalized treatment strategies to help you make positive lifestyle changes to prevent, improve, manage or even reverse certain health issues.
Our treatment plans are tailored to meet your unique needs and can address a range of conditions such as asthma, high blood pressure, diabetes and pre-diabetes, low back pain and more. We can provide one-on-one coaching, disease specific education, self-care training and condition management tactics, diet guidelines and physical activity goals, medication counseling and preventive and ongoing monitoring to manage your conditions and help you and your family stay well.
Vaccines
Vaccines are an essential part of public health, which is why we offer convenient access to vaccinations at the City of Greeley Employee Wellness Center. Your provider can determine which ones are right for members based on their age, conditions, travel habits and other factors. You can then easily schedule an appointment to get your vaccines administered.
We offer a variety of routine vaccines to protect your health and our community, including Influenza, Hepatitis A, Hepatitis B, Pneumococcal, Tdap/Td, and Zoster.
Lab services
To better support you and your family needs, we offer clinical laboratory services that include onsite testing, as well as specimen collections for offsite processing. Our turnaround time for processing routine lab tests if typically 24-48 hours. Results can be sent to you directly through the My Premise Health patient portal or discussed with your provider in a follow-up appointment, in-person or virtually.
Lab Tests that we can perform and process onsite include glucose or lipid, hemoglobin A1C, Rapid Strep, Influenza, urine pregnancy testing as well as many others.

Wellness
The City of Greeley Employee Wellness Center offers a comprehensive approach to health by focusing on overall wellness, preventive care and chronic conditions. We will treat you as a whole person, which is why our wellness programs promote nutrition and physical activity, as well as managing stress and relationships. Our wellness coaches will personalize individual wellness plans and help you and your family members make healthier choices that create positive lifestyle changes and result in a healthier you.

Wellness coaching
The City of Greeley Employee Wellness Center offers wellness coaching to help improve your overall wellbeing. Our certified wellness coaches will meet you where you are and help you make actionable goals to build new skills and make positive lifestyle changes. All coaching sessions are completely confidential and are available virtually.
Our wellness coaches address a variety of lifestyle needs including weight and stress management, nutrition coaching, physical activity routines, tobacco cessation, work/life balance and life satisfaction. They will help you reach your wellness goals through individual wellness coaching and goal setting, group education and lifestyle management programs, seminars, health fairs, meditations and podcasts. Wellness coaching is available virtually for all members, spouses and qualifying dependents.

Provider Dispensing
Skip the extra trip to the pharmacy. You can get your medications at little to no cost directly from your provider during a visit. Simply schedule an appointment, discuss your health needs and leave with your medication in hand.
The City of Greeley Employee Wellness Center carries close to 100 medications to cover your needs. This includes over-the-counter options. You can even get up to a 90-day supply for ongoing prescriptions to reduce the number of times you need a refill. When you’re about to run out, schedule a follow-up visit with your provider. Premise Health is a provider of in-person and virtual health for the world’s leading organizations. In other words, we partner with companies to bring their employees healthcare services and operate their wellness centers.
Members are what we call our patients who are eligible to use a wellness center operated by Premise Health. It’s not a membership that you have to pay for or apply to be a part of. You’re simply considered a member once your employer has partnered with us to provide your health and wellness services.
A wellness center is where you receive care as a member. We use this term versus the typical “doctor’s office” because we offer so much more than the usual office environment. Our centers not only typically offer more services than the average doctor’s office, but are also designed with you in mind, so you feel comfortable, calm and welcomed.
The City of Greeley Employee Wellness Center is staffed, managed and operated by Premise Health. Your medical staff includes a combination of physicians, nurses, physical therapists, medical assistants, registered dietitians and wellness coaches. Each staff member has been carefully screened and is qualified to deliver exceptional care. City of Greeley Employee Wellness Center is not staffed or managed by the City of Greeley or their employees in any way.

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff.
