Your designated care manager is a registered nurse who will check in with you regularly in person or virtually. They’ll be your go-to resource for health-related questions, the logistics of your care, managing your prescriptions and more.
They will act as a single point of contact between you and the rest of your care team and stand with you so that you don’t have to face ongoing health challenges on your own.

Your provider may already be supporting you in your health journey, especially if you’re managing one or more conditions such as diabetes, hypertension or osteoarthritis. Your care manager can support a broad range of needs and conditions, including mental health, lifestyle medicine and other underlying health challenges. They will work closely with your provider to design a plan that’s right for you.
You’ll receive a personalized care plan with ongoing support and follow-up as well as coaching and education to address health concerns that are impacting your life. They are there to help empower you so that you are supported in meeting your health goals.
