This benefit provides guided support to help you reach a healthier outcome more easily than you would on your own, and it comes at no cost to you.
If you or your dependent is dealing with an ongoing health challenge that has impacted overall health, caused ER or urgent care visits, hospitalizations or frequent doctor or specialist visits, we are here for you. These challenges include but are not limited to:
- Hypertension
- Hyperlipidemia
- Diabetes
- Obesity
- Metabolic syndrome
- And more

Meet your Care Manager: Your personal go-to resource
Your designated Care Manager is a Registered Nurse who will check in with you regularly in person or virtually and be your go-to resource for health-related questions, the logistics of your care, managing your prescriptions and more.
They will act as a single point of contact between you and the rest of your care team and stand with you so that you don’t have to face ongoing health challenges on your own.

Our dedicated Care Managers are registered nurses and will offer hands-on assistance and create a personalized plan that will empower you to improve your overall wellbeing.

Your provider may already be supporting you in your health journey, especially if you’re managing one or more chronic conditions such as diabetes, hypertension, or osteoarthritis. Your Care Manager can support a broad range of needs and conditions, including mental health, lifestyle medicine and other underlying health challenges. They will work closely with your provider to design a plan that’s right for you.
You’ll receive a personalized care plan with ongoing support and follow-up as well as coaching and education to address health concerns that are impacting your life. They are there to help empower you so that you are supported in meeting your health goals. Additionally, your Care Manager can help you get to the root cause of your health issues and assist you in making informed decisions while simultaneously helping you make better lifestyle choices.
