Dr. Cherukuri, known to her patients as Dr. C, is a Board-Certified Family Physician with over sixteen years of experience. Her special interests are in Preventive Medicine, Population Health, and Chronic Care Management. Dr. Cherukuri’s favorite part of her profession is building rapport with people through active listening and personalizing the care experience to facilitate shared decision-making.
Dr. Cherukuri received the degree of Fellow from the American Academy of Family Physicians, recognizing her as a champion of family medicine for her distinguished service to the community. She completed her Executive MBA at the University of Tennessee and completed the Managing Healthcare Delivery program at Harvard Business school.
Ebony has over 18 years of experience in healthcare. She has experience in multiple clinical areas, including hematology/oncology, primary care, and aesthetics. She previously worked at Parkland Hospital as a navigator, where she oversaw breast cancer and hematology clinics.
Mischele Dillard is a registered nurse, certified yoga instructor and lifelong student of lifestyle medicine. She supports members with long-term health conditions such as hypertension, obesity and diabetes. Her prior clinical experience lies in surgical services with a focus on women’s health, oncology and surgical reconstruction.
She is committed to providing a comprehensive, collaborative approach to care that serves the whole person – mind, body and spirit – while helping members get, stay, and be well.
Dr. Avila is a licensed Psychologist and a Professional counselor supervisor with experience working in both clinical and non-clinical settings. Dr. Avila holds a Ph.D in Counseling Psychology from Texas Woman’s University.
Dr. Avila is a firm believer in holistic treatment and using evidence-based interventions with a passion for creating therapeutic environments that nourish individuals and families towards growth.
Tracye has a master’s degree in social work, with over two decades of experience in the field. With 18 years dedicated to counseling, her expertise spans working with infants, teens, veterans, individuals with addictions, couples, and the elderly. Passionate about diversity and inclusivity, Tracye has worked extensively with individuals from various cultural and ethnic backgrounds, enriching her practice with a deep understanding of diverse perspectives and needs.
As a trauma-informed therapist, she prioritizes collaborative efforts to cultivate coping skills, navigate thought patterns, and foster healthier habits. With a focus on PTSD, Depression, Anxiety, and Couples therapy, she draws from a diverse toolkit of modalities including Cognitive Behavioral Therapy, Cognitive Processing Therapy, Emotion-Focused Therapy, and Solution-Focused Therapy to tailor interventions to individual needs and goals.
Originally from Olive Branch, Mississippi, Matt attended both Mississippi State University and the University of Mississippi, where he graduated with a Bachelor’s Degree in business management. Matt went on to earn his Master of Physical Therapy from California State University, Northridge. He earned his certification in Mechanical Diagnosis and therapy in 2013 and become a Certified Manual Trigger Point Therapist in 2016. He completed his Diploma in Mechanical Diagnosis and Therapy in November 2018 and fellowship with the American Academy of Manual Physical Therapy.
In his time as a physical therapist, Matt has operated several clinics in the Memphis and North Mississippi area.
Dr. Saleh is an experienced Chiropractor, skilled at helping patients reduce pain and increase mobility using joint motion, soft tissue therapy, and corrective treatments.
Dr. Saleh holds a Bachelor of Science in Business Finance from California State University, Long Beach, and a Doctor of Chiropractor degree from Southern California State University of Health Sciences in Whittier, CA.

Dr. Ezell is an experienced Chiropractor, skilled at helping patients reduce pain and increase mobility using joint motion, soft tissue therapy, and corrective treatments.
Dr. Ezell holds a Bachelor of Science in Exercise/Pre-Physical Therapy from Southern Missouri State University and a Doctor of Chiropractic degree from Parker University in Dallas.
