You don’t have to navigate your health challenges alone. With our care management services, you can get the support you need. This no-cost benefit provides a registered nurse as your care manager to be a go-to resource for all health-related questions, prescription guidance and more. They will come up with a personalized treatment plan to help you make positive lifestyle changes to prevent, improve, manage or even reverse certain health issues.
Our treatment plans are tailored to meet your unique needs and can address a range of conditions, such as diabetes, cancer, lupus, hypertension, mental health issues or any other health challenge or condition that may require a need for frequent touchpoints with your care team. We can provide personalized plans, coaching, education and ongoing monitoring and follow-ups to manage your symptoms and help you stay well.
We believe that your mental and emotional wellbeing are just as important as your physical health. With our mental health services, you can get the support you need. Your licensed counselors will actively work as part of your care team to support your overall health while making you feel safe and comfortable. You can conveniently meet with them in person or virtually, through video or by phone.
With a blended approach of counseling techniques and evidence-based talk therapy, our counseling services cover anxiety, depression, stress management, PTSD, ADHD, grief, adjustment challenges, substance abuse, work/life balance, family conflict and sleep issues.

Skip the extra trip to the pharmacy. You can get your medications at little to no cost directly from your provider during a visit. Simply schedule an appointment, discuss your health needs and leave with your medication in hand.
Premise Health Wellness Centers carry close to 200 medications to cover your needs. This includes over-the-counter options. You can even get up to a 90-day supply for ongoing prescriptions to reduce the number of times you need a refill. When you’re about to run out, schedule a follow-up visit with your provider. They’ll monitor your progress and offer ongoing support.
We understand a woman’s healthcare needs are unique. Your provider will spend time getting to know you to best understand your health goals. They’ll come up with a personalized care plan to support your long-term health.

Our turnaround time for processing routine lab tests is typically 24 to 48 hours. There may be times you need specialty care outside of our center. Finding that care on your own can sometimes be confusing. That’s why we offer care navigation services to support you in finding the affordable, high-quality care you need.
If your provider makes a specialist referral, our care navigation team will help you find the best care at the best price in your community. They’ll even assist with scheduling your appointments, transferring your medical records and following up after your visits to help with any ongoing care needs.

Wellness coaching can help you achieve lasting lifestyle change in the areas of weight management, nutrition, physical activity, sleep, stress, smoking cessation, work/life balance and life satisfaction. Our certified wellness coaches are trained specialists who support you as you move toward better health at a comfortable pace. They will help you set actionable goals and teach you new skills as you learn to incorporate healthier habits into your daily routine. All coaching sessions are completely confidential and can take place virtually or in person.
Eating well can improve your overall wellbeing. It can also help treat existing conditions and decrease future risk. At Chick-fil-A Wellness Center, your registered dietitians use medical nutrition therapy to help treat a range of conditions, such as diabetes, hypertension, cardiovascular disease, as well as many digestive disorders and weight-related issues. They will tailor your nutrition counseling to meet you and your family’s unique needs and lifestyle with personalized action plans, based on your risk factors and family history.
Our registered dietitians can provide advice and ongoing education, in-person or virtually through one-on-one tailored sessions and individual nutrition assessments, group nutrition classes, lunch and learns, health fairs, seminars, podcasts, self-guided nutrition tools and virtual dietitian guided appointments, ages 18 and up, to give you and your family a new appreciation for healthy food, and in turn, improve your overall health.

We are here as your health ally to help you get, stay and be well. Members are what we call our patients. If your employer has partnered with Premise Health to provide access to our wellness centers and services, you’re automatically considered an eligible member. There is no membership fee to get started. It also reflects how much we value our care relationship with you, because to us, you’re more than just a patient – you’re a member.
A wellness center is where you receive care as a member. We use this term versus the typical “doctor’s office” because we offer so much more than the usual office environment. Our centers not only typically offer more services than the average doctor’s office, but are also designed with you in mind, so you feel comfortable, calm and welcomed.
The center is staffed, managed and operated by Premise Health. The medical staff includes a combination of physicians, nurses, physical therapists, medical assistants, registered dietitians, wellness coaches and fitness specialists. Each staff member has been carefully screened and is qualified to deliver exceptional care. The center is not staffed or managed by Chick-fil-A or their Staff in any way.
My Premise Health is your member portal. It is a secure online and mobile tool that you can use to manage your health information. My Premise Health connects you to your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

All preventive services, nutrition counseling, wellness coaching, virtual behavioral health and fitness are no cost. Anthem or BCBS HDHP members will meet deductible for acute office visits, behavioral health, and physical therapy. After the deductible, the cost is $30.00 for office visits and behavioral health. Physical Therapy will be a 10% co-insurance until the out-of-pocket maximum is met. Virtual Primary Care (nationwide coverage) is offered at $15.00 per visit. Anthem and BCBS PPO have a $30.00 copay for office visits.

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. Yes, you may request any records be sent to another medical provider. If your community provider uses Epic (or My Chart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.

A virtual visit is just like an in-person appointment. You can discuss your medical needs with your provider, and they will diagnose and prescribe medications as needed.
Virtual visits are designed to treat your short-term needs and provide long-term preventive care with convenience and flexibility. If you’re experiencing a medical emergency, please dial 911 as soon as possible to get the medical attention you need.

Preventive care includes routine checkups and screenings that help you stay healthy in the long-term. These are visits you make when you’re feeling well to catch any health issues early or improve your wellbeing. This includes annual physicals, cancer screenings, vaccinations and more.
Non-preventive care on the other hand, includes visits you schedule when you’re not feeling well. These are meant to address your symptoms to find out what’s wrong and get you back to feeling your best. Some examples include sick visits and treatment for issues like cold and flu, allergies, injuries and more.
Yes, your wellness center can perform blood draws and other lab tests ordered by an outside provider. All lab orders must be faxed or dropped off at the center to ensure we have the proper supplies for the required testing.
