We offer a comprehensive range of primary care services that focus on your long-term health, including sick and preventive care and management of your conditions. Your primary care provider will get to know you and your family, so they are better equipped to prevent, treat, and manage any health issues that may come up.
Our lifestyle medicine approach ensures we’re looking at the whole picture of what impacts your wellbeing, including sleep, nutrition, stress, movement and more. We can encourage healthy habits to help you achieve sustainable wellness and go beyond simply treating the symptoms you bring with you to a single appointment.
You don’t have to navigate your health challenges alone. With our care management services, you and your family can get the support you need. This no-cost benefit provides a registered nurse as your care manager to be a go-to resource for all health-related questions, prescription guidance and more. They will come up with a personalized treatment plan to help you make positive lifestyle changes to prevent, improve, manage or even reverse certain health issues.
Our treatment plans are tailored to meet your unique needs and can address a range of conditions, such as asthma, high blood pressure, diabetes and pre-diabetes, low back pain, and more. We can provide one-on-one coaching, disease-specific education, self-care training and condition management tactics, diet guidelines and physical activity goals, medication counseling, and preventive and ongoing monitoring to manage your symptoms and help you and your family stay well.
Skip the extra trip to the pharmacy. You can get your medications at little to no cost directly from your provider during a visit. Simply schedule an appointment, discuss your health needs and leave with your medication in hand.
The ECHO Health Center carries close to 200 medications to cover your needs. This includes over-the-counter options. You can even get up to a 90-day supply for ongoing prescriptions to reduce the number of times you need a refill. When you’re about to run out, schedule a follow-up visit with your provider. They’ll monitor your progress and offer ongoing support.
We understand a woman’s healthcare needs are unique. Your provider will spend time getting to know you to best understand your health goals. They’ll come up with a personalized care plan to support your long-term health.

Wellness coaching can help you achieve lasting lifestyle change in the areas of weight management, nutrition, physical activity, sleep, stress, smoking cessation, work/life balance and life satisfaction. Our certified wellness coaches are trained specialists who support you as you move toward better health at a comfortable pace. They will help you set actionable goals and teach you new skills as you learn to incorporate healthier habits into your daily routine. All coaching sessions are completely confidential and can take place virtually.
Our wellness programs promote nutrition, physical activity, stress management, healthy relationships and more. We take a comprehensive approach that focuses on your overall health and wellbeing. Our providers will personalize your wellness plan and help you make positive lifestyle changes that can lead to a more balanced and healthier you.
Your wellness services include biometric screenings, wellness coaching, medication consultations, and condition management. The ECHO Health Center is also partnered with the Town of Smyrna’s wellness program. Ask us how we can assist you with appointments and referrals to this great program
We offer many lab services to support you and your family’s needs. We can perform onsite testing and can also collect and send samples to an offsite lab. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health account or during an in-person or virtual follow-up appointment with your provider.

The ECHO Health Center is staffed, managed, and operated by Premise Health. The medical staff includes a combination of physicians, nurse practitioners, nurses, and wellness coaches. Each staff member has been carefully screened and is qualified to deliver exceptional care. Your wellness center is not staffed or managed by the Town of Smyrna or its employees in any way.
My Premise Health is your member portal. It is a secure online and mobile tool that you can use to manage your health information. My Premise Health connects you to your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

All Town of Smyrna employees and dependents ages 2 years and up that are participating in a Town of Smyrna health plan are eligible to use wellness center services.
There is no cost for services at the ECHO Health Center. If you are participating in the Town of Smyrna HSA health plan, the IRS requires you to pay a small copay for acute need provider appointments and medications.

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.
Yes, you may request any records be sent to another medical provider. If your community provider uses Epic (or My Chart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.

To meet virtually with one of the providers at your wellness center, you must first create an account on the My Premise Health app or online at mypremisehealth.com. A virtual visit is just like an in-person appointment. You can discuss your medical needs with your provider, and they will diagnose and prescribe medications as needed.

While the ECHO Health Center does not provide full pediatric services, we can provide acute pediatric care for children ages 2 years and up, which can include care for cold and flu, and school and sports physicals.
