$30 charge at time of service for associate (applied to deductible, billed for remainder)
$10 charge for dependent visits

Benefits under this Family Health Center Program are generally the same as under the prescription drug coverage portion of the Medical and Prescription Drug Program in which you are enrolled. The DENSO Family Pharmacy can fill prescriptions written for up to a 90-day supply. Various immunizations such as influenza, shingles, and pneumococcal are available at no cost to eligible Associates or their eligible Dependents. See the DENSO Family Pharmacy staff for availability and restrictions on administration.

OpenRoad Medical Program Option Only:

Individuals who are enrolled in the OpenRoad Medical Program option may also utilize the DENSO Family Pharmacy, but are still required to pay the deductible and coinsurance as required under the SPD. OpenRoad Medical Program option enrollees will be charged reasonable and customary pricing for services received from the DENSO Family Pharmacy. Please consult the SPD for additional information.
